本文对多梗塞性痴呆(MID)患者的头颅CT图像作双额径(BF)、双尾径(BC)及Ⅲ脑室外侧至半球外侧裂径(ⅢV-S)与其相应颅腔径比率测定。结果表明MID组的BC比率增加,ⅢV-S比率减少,与健康对照组及脑梗塞非痴呆组比较,差异有显著性意义。本文认为多梗塞性痴呆不论梗塞灶的部位和容量大小,其痴呆的产生与Ⅲ脑室外侧壁、丘脑和脑岛之间的结构损害累及丘脑及其投射和Papez环路有关。同时,根据病史、症状和体征结合头颅CT检查有助于鉴别腔隙状态所致的痴呆、Alzheimer氏病的痴呆,以及多发性脑梗塞合并Alzheimer氏病。
In this paper, the head CT images of patients with multi-infarct dementia (MID) were measured by double-quotient (BF), two-tail (BC) and Ⅲ lateral ventricle to lateral hemisphere (ⅢV-S) The results showed that the BC group increased the proportion of BC, Ⅲ V-S ratio decreased compared with the healthy control group and non-infarction group of cerebral infarction, the difference was statistically significant. This article suggests that multi-infarct dementia regardless of infarct size and size of the site, its dementia and Ⅲ lateral ventricular wall, thalamus and insula structure damage involving the thalamus and its projection and Papez loop related. At the same time, combined with head CT examination on the basis of medical history, symptoms and signs can help identify the dementia caused by lacunar state, dementia of Alzheimer’s disease and multiple cerebral infarction with Alzheimer’s disease.
