目的:探索超声吸引器在乳腺癌改良根治术腋窝清扫中的临床应用。方法:将瑞金医院2005年1月至2008年9月行乳腺癌改良根治术的病人随机分为超声吸引辅助清扫组(204例)与常规清扫组(191例),观察手术时间、术后引流量、放置引流管天数、术后住院天数、淋巴结清扫数目、淋巴结转移数目及术后皮下积液等并发症。结果:超声吸引辅助清扫组淋巴结清扫数目较常规清扫组显著增多(P<0.001),术后引流量、引流管放置天数、住院天数均比常规手术组显著减少(P<0.001),手术时间、淋巴结转移数目两组无统计学差异。结论:超声吸引器在腋窝中使用安全、术野清晰,淋巴结清扫较彻底,术后引流量和置管天数减少,术后皮下积液等并发症少,在乳腺癌手术腋窝清扫中有其临床应用价值。
Objective: To explore the clinical application of ultrasonic aspirator in axillary dissection of modified radical mastectomy for breast cancer. Methods: Patients with modified radical mastectomy of breast cancer from January 2005 to September 2008 in Ruijin Hospital were randomly divided into two groups (204 cases) and routine cleaning group (191 cases). The operative time, Flow, the number of days the drainage tube was placed, the number of postoperative hospitalizations, the number of lymph node dissections, the number of lymph node metastases and postoperative subcutaneous effusion. Conclusion: Ultrasound aspirator is safe to use in the armpit, with clear operative field, thorough lymphadenectomy, less postoperative drainage and days of catheterization, fewer postoperative subcutaneous effusions and so on. It has clinical application in axillary dissection of breast cancer Value.
