目的 探讨结扎肝硬化中国小型猪肝右静脉后对其门静脉压力 (PVP)的影响。方法 12只中国小型猪建立肝硬化模型后 ,戊巴比妥钠腹腔麻醉下剖腹 ,B超介导下结扎肝右静脉 ,选取一支大网膜静脉置管测量结扎前及结扎后 30min及 1,3 ,5 ,7,14 ,2 1,2 8,5 6d的PVP。结果 剖腹手术均顺利完成 ,手术后均发生PVP升高 ,6只小猪术后PVP超过 35cmH2 O ,且其中 4只小猪出现上消化道出血 ,呈自限性。术中术后无一例死亡。结论 肝硬化条件下结扎肝右静脉会引起PVP早期升高 ,后期呈下降趋势 ;压力过高者会造成早期上消化道出血。一般无结扎肝叶的坏死
Objective To investigate the effect of ligation of the right hepatic vein of Chinese miniature pigs on portal pressure (PVP). Methods Twelve Chinese miniature pigs were induced with hepatic cirrhosis by intraperitoneal anesthesia. The right hepatic vein was ligated by B-mode ultrasound. One large omentum vein catheter was used to measure the blood pressure before and after ligation 30min and 1 , 3, 5, 7, 14, 2 1, 2, 8, 5 6d. Results Laparotomy was completed successfully. PVP increased after operation. The PVP of 6 piglets exceeded 35cmH2O after operation, and 4 piglets showed upper gastrointestinal bleeding with self-limiting. No one died after surgery. Conclusions Ligation of the right hepatic vein under cirrhosis can lead to an early increase in PVP and a decrease in late period. Overpressure can cause early upper gastrointestinal bleeding.
