目的研究PASS直丝弓矫治器治疗上颌第一恒磨牙近中阻生的临床效果。方法选择唐山市口腔医院替牙期上颌第一恒磨牙近中倾斜压迫上颌第二乳磨牙远中根并有不同程度吸收的患者25例,男15例,女10例;年龄7~11岁,平均9岁零2个月,替牙列,拔出上颌第二乳磨牙,让上颌第一恒磨牙自行萌出,呈近中倾斜状态。第一阶段PASS直丝弓矫治器结扎0.012Ni-Ti弓丝轻力直立上颌第一恒磨牙;第二阶段0.016澳丝保持第一阶段结果并Ni-Ti推簧推上颌第一恒磨牙向远中,恢复到正常位置。结果所有患者上颌第一恒磨牙牙冠均向远中移动(PTV-U6)4.7mm(P<0.05),每侧可提供约5mm间隙供给上颌第二前磨牙萌出,上颌第一恒磨牙远中倾斜(SN-U6)8.2°(P<0.05),上前牙唇侧倾斜(SN-U1)0.5°(P<0.05),上下磨牙达到中性咬合关系或中性偏近中咬合关系,治疗时间4.0~6.0个月,平均5.1个月。结论 PASS直丝弓矫治器是治疗上颌第一恒磨牙近中阻生的有效方法。
Objective To study the clinical effect of PASS straight wire appliance in the treatment of proximal maxillary impaction of maxillary first permanent molars. Methods Totally 25 patients (25 males and 15 females) were enrolled in Tangshan Stomatological Hospital for maxillary first permanent molars with moderate to moderate lateral incisors and maxillary second molars. Results All the patients’ maxillary first permanent molars had distal displacement (PTV-U6) of 4.7 mm (P <0.05), and each side could provide about 5 mm gap for maxillary second premolar eruption. (SN-U6) was 8.2 ° (P <0.05), the anterior lip was inclined 0.5 ° (P <0.05), the upper and lower molars reached neutral occlusion or neutral occlusion, the treatment time 4.0 ~ 6.0 months, an average of 5.1 months. Conclusion PASS straight wire appliance is an effective method to treat proximal maxillary impaction of the maxillary first permanent molars.
