目的探讨64层螺旋CT灌注成像(CT perfusion,CTP)在超急性脑梗死患者非梗死侧脑组织血流动力学中的应用。资料与方法采用GE Lightspeed VCT(64层螺旋CT)机对超急性脑梗死组17例、正常组11例行脑CTP,将图像传送到ADW4.3工作站,使用CT Perfusion 3软件包进行处理,测量所选脑内感兴趣区(ROI)的脑血流量(CBF)、脑血容量(CBV)及平均通过时间(MTT),并进行单因素方差分析。结果超急性脑梗死组非梗死侧脑灰质的CBF、CBV、MTT分别为(49.2±10.8)ml.100 g-1.min-1、(2.4±0.7)ml/100 g、(3.2±0.8)s,豆状核分别为(61.3±14)ml.100 g-1.min-1、(2.5±0.6)ml/100 g、(2.7±0.4)s,脑白质分别为(22.2±8.1)ml.100 g-1.min-1、(1.3±0.2)ml/100 g、(4.7±1.8)s;正常组左侧脑灰质的CBF、CBV、MTT分别为(46.9±7.6)ml.100g-1.min-1、(2.4±0.4)ml/100 g、(3.5±0.8)s,豆状核分别为(50.2±10.4)ml.100 g-1.min-1、(2.3±0.7)ml/100 g、(2.6±0.5)s,脑白质分别为(21.3±5.5)ml.100 g-1.min-1、(1.5±0.4)ml/100 g、(4.3±0.9)s;正常组右侧脑灰质的CBF、CBV、MTT分别为(46.5±9.3)ml.100 g-1.min-1、(2.3±0.4)ml/100 g、(3.5±0.7)s,豆状核分别为(55.8±11.08)ml.100 g-1.min-1、(2.4±0.7)ml/100 g、(2.6±0.5)s,脑白质分别为(23.2±7.0)ml.100 g-1.min-1、(1.5±0.4)ml/100 g、(4.7±1.0)s;超急性脑梗死患者非梗死侧所测各参数值与正常组之间差异无统计学意义(P>0.05)。结论超急性脑梗死患者非梗死侧CBF、CBV及MTT的波动尚在正常范围内。
Objective To investigate the application of 64-slice spiral CT perfusion imaging (CTP) in hemodynamics of non-infarcted lateral brain tissue in patients with hyperacute cerebral infarction. Materials and Methods 17 cases of hyperacute cerebral infarction were treated by GE Lightspeed VCT (64-slice spiral CT) machine, 11 cases of normal group were treated with brain CTP, the images were sent to ADW4.3 workstation and processed by CT Perfusion 3 software package. Selected brain regions of interest (ROI) of cerebral blood flow (CBF), cerebral blood volume (CBV) and mean transit time (MTT), and one-way analysis of variance. Results The levels of CBF, CBV and MTT in the non-infarcted side of gray matter in hyperacute infarction group were (49.2 ± 10.8) ml.100 g-1.min-1, (2.4 ± 0.7) ml / 100 g and (3.2 ± 0.8) s, respectively. The levels of lentiform nuclei were (61.3 ± 14) ml.100 g-1.min-1, (2.5 ± 0.6) ml / 100 g and (2.7 ± 0.4) . The levels of CBF, CBV and MTT in the left cerebral gray matter in the normal group were (46.9 ± 7.6) ml and 100g-1, respectively (2.3 ± 0.7) ml and (± 0.3 ± 0.7) ml, respectively. The levels of lentiform nucleus were (50.2 ± 10.4) ml.100 g-1.min- / 100 g, (2.6 ± 0.5) s, white matter were (21.3 ± 5.5) ml.100 g-1.min-1, (1.5 ± 0.4) ml / 100 g, The levels of CBF, CBV and MTT in the right gray matter were (46.5 ± 9.3) ml.100 g-1.min-1, (2.3 ± 0.4) ml / 100 g and (3.5 ± 0.7) (55.8 ± 11.08) ml.100 g-1.min-1, (2.4 ± 0.7) ml / 100 g, (2.6 ± 0.5) s, and white matter were (23.2 ± 7.0) ml.100 g-1min -1, (1.5 ± 0.4) ml / 100 g and (4.7 ± 1.0) s, respectively. There was no significant difference between the non-infarcted side and the normal group in patients with hyperacute cerebral infarction (P> 0.05). Conclusion The fluctuation of CBF, CBV and MTT in non-infarcted side of patients with hyperacute cerebral infarction is still within the normal range.
