目的研究该院创二甲过程中门诊医师处方特点,并采取持续干预措施降低不合理用药。方法处方选取我院2013年4月-2014年3月门诊处方120 000张,每月随机抽取10 000张门诊处方,按《处方管理办法》和《北京市医院处方点评规范》实时进行处方点评,点评内容包括处方的基本要素,包括处方用药剂量、使用疗程、适应证、药物相互作用等信息。结果 120 000张处方中不合理处方1 329张,占1.11%,包括不规范处方1 158张,不适宜处方20张,超常处方151张。结论处方点评和干预措施明显的改善了处方质量加强临床药师的药学专业和临床专业知识,进一步提高不合理处方的发现率。
Objective To study the prescribing characteristics of outpatient physicians during the creation of the Second A in the hospital and to take continuous interventions to reduce unreasonable use of drugs. Methodology Prescriptions 120,000 outpatient prescriptions were collected from April 2013 to March 2014 in our hospital. A total of 10 000 outpatient prescriptions were randomly selected every month. The prescriptions were reviewed in real time according to the “prescription management approach” and “Beijing hospital prescription review specifications” Comments include the basic elements of the prescription, including the prescription dose, the use of treatment, indications, drug interactions and other information. Results Of the 120 000 prescriptions, 1 329 were unreasonable prescriptions, accounting for 1.11%, including 1 158 non-standard prescriptions, 20 unfit prescriptions and 151 supernormal prescriptions. Conclusions The prescription reviews and interventions have significantly improved the quality of prescriptions to enhance the pharmacy professional and clinical expertise of clinical pharmacists and further improve the detection rate of unreasonable prescriptions.
