目的调查上海市嘉定区真新地区高血压合并糖尿病老年患者用药安全现况及影响因素。方法采取整群抽样的方法,调查上海市嘉定区真新地区15个社区398例60~75岁高血压合并糖尿病老年患者的用药情况。结果患者平均年龄(67.1±4.5)岁,平均患病6种,平均用药9种,药品不良反应(ADR)发生率为32.16%。与用药安全性总分明显相关的影响因素有3个,分别是文化程度、家庭经济收入和曾从事职业;药品知识了解情况得分的影响因素有2个,分别是文化程度和曾从事职业;自我药疗情况得分的影响因素有3个,分别是文化程度、家庭经济收入和曾从事职业;药学人员服务得分的影响因素有3个,分别是有无配偶、文化程度和职业。结论高血压合并糖尿病老年患者多重用药现象普遍,不良反应发生率高,存在用药安全隐患,深入开展社区药学服务有助于提高老年患者的用药安全性。
Objective To investigate the current situation and influential factors of drug safety in elderly patients with hypertension complicated with diabetes in Zhenxin District, Jiading District of Shanghai. Methods A cluster sampling method was used to investigate the medication of 398 elderly patients aged 60-75 years with hypertension and diabetes mellitus in 15 communities in Zhenxin New Area, Jiading District, Shanghai. Results The average age of patients (67.1 ± 4.5) years, with an average of 6 kinds of disease, the average medication 9 kinds of drug adverse reactions (ADR) was 32.16%. There are three influencing factors that are obviously related to the total score of drug safety, which are education level, family income, and career. There are two influencing factors for the score of drug knowledge, which are education level and career, self There are three influencing factors for the score of medical treatment, which are educational level, family economic income and career. There are three influencing factors for the score of service of pharmacy, including the spouse, education level and occupation. Conclusions Multi-drug use is common in elderly patients with hypertension and diabetes mellitus. The incidence of adverse reactions is high, and there are potential safety problems in drug use. Further intensive community pharmacy services can help improve the safety of drug use in elderly patients.
