目的研究改良式Forsus矫治器对安氏Ⅱ类错下颌后缩患者颞下颌关节的影响。方法选择2009—2011年来济南市口腔医院正畸科就诊的采用改良式Forsus矫治器治疗的安氏Ⅱ类下颌后缩患者20例,应用改良式Forsus矫治器进行功能性矫形治疗,并对治疗前后颞下颌关节变化进行测量分析。结果改良式Forsus矫治器治疗6~8个月后,髁状突后上方出现增生迹象。髁状突与关节窝的位置关系、关节盘与关节窝的位置关系治疗前后差异均有统计学意义(P<0.05);而髁状突与关节盘位置关系治疗前后差异无统计学意义(P>0.05)。结论改良式Forsus矫治器治疗安氏Ⅱ类错过程中,可刺激髁状突、关节盘产生适应性改建,髁状突、关节盘、关节窝的相对位置关系可保持相互协调。
Objective To study the effect of modified Forsus appliance on temporomandibular joint in patients with Class Ⅱ malocclusion following mandibular retrusion. Methods Twenty patients with Class Ⅱ mandibular retrusion treated with modified Forsus appliance in orthodontics department of Jinan Stomatological Hospital from 2009 to 2011 were selected. Functional orthodontic treatment was performed with modified Forsus appliance. Before and after treatment Temporomandibular joint changes were measured and analyzed. Results The modified Forsus appliance showed signs of hyperplasia after 6 to 8 months of treatment. The relationship between the position of condyle and glenoid fossa and the position of joint disc and glenoid fossa before and after treatment were statistically significant (P <0.05), while there was no significant difference between the position of condyle and disc > 0.05). Conclusion The modified Forsus appliance can induce condyle and joint disc adaptations during the treatment of Class Ⅱ malocclusion. The relative position of condyle, disc and socket can be kept in harmony with each other.
