目的:观察经皮穴位电刺激(TEAS)治疗对IVF肾气虚型患者冻胚移植周期子宫内膜容受性的影响。方法:将我院收集的153例符合纳入标准的行冻胚移植的肾气虚型患者随机分为TEAS治疗组、MOCK组、对照组。观察3组患者治疗前后临床指标的变化,以及妊娠结局的差异。结果:3组患者的平均年龄、不育年限、体重指数、基础FSH比较差异无统计学意义。治疗后,TEAS组患者中医证候评分较治疗前明显降低,差异有统计学意义(P<0.05)。黄体酮注射日TEAS组的子宫内膜厚度高于MOCK组和对照组,差异有统计学意义(P<0.05);TEAS组A型与AB型子宫内膜百分比、(+、++)型子宫内膜下血流明显高于MOCK组和对照组,差异有统计学意义(P<0.05);黄体酮注射日3组血清E2、P比较,差异无统计学意义(P>0.05);TEAS组PI、RI、S/D低于治疗前,与MOCK组、对照组比较明显降低,差异均有统计学意义(P<0.05),治疗前后MOCK组与对照组两组间、组内比较差异均无统计学差异(P>0.05)。TEAS组HCG阳性率及临床妊娠率明显高于MOCK组与对照组,差异有统计学意义(P<0.05)。MOCK组与对照组比较无统计学差异(P>0.05)。结论:TEAS治疗能改善IVF肾气虚型患者的中医临床症状及体征,增加子宫内膜厚度,降低子宫内膜血流参数PI、RI、S/D,进而提高患者的子宫内膜容受性,改善临床妊娠结局。
Objective: To observe the effect of transcutaneous electroacupuncture (TEAS) on the endometrial receptivity of frozen embryo transfer in IVF patients with kidney qi deficiency. Methods: A total of 153 patients with renal deficiency syndrome who were included in our hospital were randomly divided into TEAS treatment group, MOCK group and control group. The changes of clinical indexes and the differences of pregnancy outcomes before and after treatment were observed in 3 groups of patients. Results: There was no significant difference in average age, infertility duration, body mass index and basal FSH between the three groups. After treatment, the score of TCM syndrome in TEAS group was significantly lower than that before treatment (P <0.05). The endometrial thickness of TEAS group on progesterone injection day was higher than that of MOCK group and control group (P <0.05). The percentage of type A and AB endometrium, the type of uterus (+, ++) in TEAS group The subendocardial blood flow was significantly higher in MOCK group than in control group (P <0.05). There were no significant differences in serum E2 and P between the three groups on the day of progesterone injection (P> 0.05) PI, RI and S / D were lower than those before treatment, and were significantly lower than those in MOCK group and control group (P <0.05). Before and after treatment, there was significant difference between the two groups in MOCK group and control group No statistical difference (P> 0.05). The HCG positive rate and clinical pregnancy rate in TEAS group were significantly higher than those in MOCK group and control group (P <0.05). There was no significant difference between MOCK group and control group (P> 0.05). Conclusion: TEAS treatment can improve the clinical symptoms and signs of traditional Chinese medicine in IVF patients with qi deficiency, increase the thickness of endometrium, decrease the endometrial blood flow parameters PI, RI and S / D, and then improve the endometrial receptivity, Improve clinical pregnancy outcomes.
