目的观察不同程度三尖瓣反流(TR)对三尖瓣环收缩期峰值流速(RV-Sm)、运动幅度(TAMA)与右室射血分数(RVEF)相关关系的影响。方法110例TR患者按反流程度分轻、中、重三组,接受超声心动图检查。其中30例(每组10例)进行了MRI右室造影。结果超声Simpson法RVEF与MRI法RVEF呈显著正相关。轻、中度TR组,RV-Sm与RVEF相关性良好(r分别为0.76,0.75),重度TR组RV-Sm与RVEF无相关性(r=0.2)。随着TR加重,TAMA出现了与RV-Sm一致的变化。结论重度TR影响三尖瓣环运动。重度TR时RV-Sm和TAMA不能反映RVEF。
Objective To observe the effect of tricuspid regurgitation (TR) on the relationship between RV-Sm, TAMA and right ventricular ejection fraction (RVEF) in tricuspid annulus. Methods One hundred and ten patients with TR were divided into mild, moderate and severe groups according to the degree of reflux, and were examined by echocardiography. Thirty of them (10 in each group) underwent MRI right ventricular angiography. Results Ultrasound Simpson method RVEF and MRI RVEF was significantly correlated. There was a good correlation between RV-Sm and RVEF in mild and moderate TR patients (r = 0.76 and 0.75, respectively) and no correlation between RV-Sm and RVEF in severe TR patients (r = 0.2). As TR increased, TAMA showed a consistent change with RV-Sm. Conclusion Severe TR affects the movement of tricuspid annulus. RV-Sm and TAMA do not reflect RVEF at severe TR.
