目的分析原因不明复发性流产(unexplained recurrent spontaneous abortion,URSA)患者外周血中Th1/Th2细胞的表达,探讨免疫因子平衡失调在URSA发病机制中的作用。方法 70例URSA患者为URSA组,正常早孕者70例为早孕组,正常未孕者70例为未孕组,采用ELISA法测定3组血白细胞介素(interleukin,IL)-2、干扰素(interferon,IFN)-α、IFN-γ以及IL-4、IL-10表达水平;以未孕组细胞因子表达水平为标准值,对URSA组和早孕组细胞因子变化值进行统计学分析。结果 URSA组IL-2[(8.37±0.57)μg/L]、IFN-α[(31.57±1.06)μg/L)、IFN-γ[(694.15±43.71)μg/L]高于早孕组[(4.18±0.49)、(5.41±0.91)、(506.19±58.23)μg/L]及未孕组[(3.98±0.61)、(5.78±0.83)、(468.39±41.58)μg/L),IL-4[(11.73±2.11)μg/L]、IL-10[(16.86±1.36)μg/L]低于早孕组[(26.18±3.68)、(28.16±4.16)μg/L]及未孕组[(25.39±2.81)、(27.72±3.48)μg/L],差异均有统计学意义(P<0.01);早孕组与未孕组各细胞因子表达水平比较差异均无统计学意义(P>0.05)。结论母体Th1细胞因子高表达、Th2细胞因子低表达与URSA发生有关,免疫平衡失调可能是介导URSA的原因之一。
Objective To analyze the expression of Th1 / Th2 in peripheral blood of patients with unexplained recurrent spontaneous abortion (URSA), and to explore the role of immune imbalance in the pathogenesis of URSA. Methods Seventy URSA patients were in the URSA group. 70 normal pregnant women were in the early pregnancy group and 70 normal pregnant women were in the non-pregnant group. The levels of interleukin (IL) -2, interferon (IL-2) interferon, IFN-α, IFN-γ, IL-4 and IL-10. The cytokine levels in URSA group and early pregnancy group were statistically analyzed. Results The levels of IL-2 in URSA group were significantly higher than those in the first trimester group [(8.37 ± 0.57) μg / L], IFN-α [(31.57 ± 1.06) μg / L) and IFN- γ [(694.15 ± 43.71) (4.18 ± 0.49), (5.41 ± 0.91) and (506.19 ± 58.23) μg / L, respectively, and no pregnancy group (3.98 ± 0.61 and 5.78 ± 0.83 and 468.39 ± 41.58 μg / L, respectively) [(11.73 ± 2.11) μg / L] and IL-10 [(16.86 ± 1.36) μg / L] were lower than those in early pregnancy [(26.18 ± 3.68) and (28.16 ± 4.16) μg / 25.39 ± 2.81), (27.72 ± 3.48) μg / L], the differences were statistically significant (P <0.01). There was no significant difference in the expression of cytokines between the early pregnancy group and the non-pregnant group (P> 0.05) . Conclusion The high expression of Th1 cytokines in maternal and the low expression of Th2 cytokines are associated with the occurrence of URSA. Immune imbalance may be one of the reasons of URSA mediated.
