本文研究了由手术证实的356例选择性右室负荷过重型先心病儿童心电向量图。按病种共分9个类型。术前常规记录Frank导联心电向量图。测量和统计了F、LS及H面QRS环在各象限的面积比率,用本室相应年龄正常人心电向量图作对照组。进行了数理统计及诊断试验研究。结果表明:诊断先心病右室肥厚最有价值的指标是H2大于总面积15%、F2大于总面积30%及F1小于总面积50%。用QRS环面积法诊断右室肥厚的敏感性、特异性、准确性、阳性似然比及阳性诊断价值分别为60.0%~88.3%、92.3%~95.8%、90.9%~95.1%、8.7~20.5及89.7%~95.5%。试验证明,QRS环面积是诊断右室肥厚最简易而可靠的定量诊断方法。
In this paper, 356 cases of selective right ventricular hypertrophy of children confirmed by surgery cardiogram ECG. According to the disease is divided into 9 types. Preoperative routine recording Frank lead ECG vector diagram. Measurement and statistics of the F, LS and H surface QRS ring in the quadrant area ratio, with normal room age room corresponding ECG vector diagram for the control group. The results show that: the most valuable indicators of diagnosis of congenital right ventricular hypertrophy H2 is greater than the total area of 15%, F2 is greater than 30% of the total area and F1 less than 50% of the total area. The sensitivity, specificity, accuracy, positive likelihood ratio and positive diagnostic value of QRS annulus area in the diagnosis of right ventricular hypertrophy were 60.0% ~ 88.3%, 92.3% ~ 95.8%, 90 .9% ~ 95.1%, 8.7 ~ 20.5 and 89.7% ~ 95.5%. Experiments show that, QRS ring area is the most simple and reliable diagnosis of right ventricular hypertrophy quantitative diagnosis.
