目的 通过分析煤矿职工的肺功能检查结果, 探讨影响煤矿职工肺功能异常的影响因素, 为煤工尘肺的早期预防提供参考依据.方法 随机抽取某煤矿集团3个矿井共6 536名接触粉尘作业的劳动者作为研究对象, 按照职业病危害作业等级进行分级, 分析职业健康检查结果中肺功能检查情况, 并对肺功能异常情况进行多因素Logistic回归分析.结果 山西省某煤矿6 536名劳动者中, 肺功能异常2 019例, 占30.9%;年龄、吸烟情况、烟龄、工龄及粉尘作业等级是其肺功能异常的影响因素 (P<0.05);多因素Logistic回归分析结果显示, 接尘工龄为10~20年、中度危害作业、高度危害作业是肺功能异常的危险因素 (OR值分别为1.410、2.875、2.375, P<0.05), 偶尔佩戴和经常佩戴是肺功能异常的保护因素 (OR值分别为0.444、0.454, P<0.05) .结论 煤矿职工肺功能异常与劳动者的职业病危害作业等级、接尘工龄有关, 戴防尘口罩可有效降低肺功能异常率.“,”Objective To analyze the pulmonary function testing data of dust exposed workers in a coal mine industry group, observe the adverse effects of coal dust on pulmonary function and provide orientation for early prevention of pneumoconiosis.Methods A total of 6 536 workers exposed to coal dust in 3 mines were randomly selected as the research object, and be graded by the classification of occupational examination.The occupational health examination results were graded according to the occupational hazards, the pulmonary function examination results were analyzed.Multivariate logistic regression analysis was conducted to analyzed the abnormal pulmonary function.Results Among the 6 536 workers in a coal mine in Shanxi, there were 2 019 cases of abnormal lung function, accounting for 30.9%.Age, smoking status, smoke age, dusting age and dust operation level were the influencing factors for pulmonary dysfunction (P<0.05).Multivariate logistic regression analysis confirmed that the dusting age of 10~20 years (OR=1.410, P<0.05), moderate hazard operation (OR=2.875, P<0.05), high-hazard operation (OR=2.375, P<0.05) were the risk factors of abnormal lung function, but occasional wear (OR=0.444, P<0.05) and frequent wear (OR=0.454, P<0.05) were protective factors.Conclusion The pulmonary function of coal mine workers is related to the classification of occupational hazards and dust exposure working age.Wearing dust protecting mask can effectively reduce the abnormal lung function rate.
