目的　研究改良根治术联合碘 - 12 5粒子近距离照射治疗乳腺癌的近期疗效。方法　 2 0 0 2年 6月至 2 0 0 3年 11月 ,2 6例乳腺癌患者行改良根治术中 ,于胸骨旁后穿刺 ,沿胸廓内动脉周围纵形顺序置入碘 -12 5粒子 ,并于锁骨上下分别置入碘 - 12 5粒子 ,术后观察切口及创面愈合情况 ,复查胸片、血象及肝肾功能 ,系统查体检查有无局部复发及转移。结果　随访 3～ 2 0月 (平均 10月 )。未见与植入碘 - 12 5有关之并发症。至目前尚未发现局部复发 ,2例病人有远处转移。术后全身反应小 ,无白血球、血小板下降、无肝肾功能受损。结论　术中置入碘 - 12 5粒子对乳腺癌术后切口愈合无不良影响 ;靶点准 ,全身反应轻 ,可于术后及时开始化疗等辅助治疗 ;可持续放疗 ,更符合癌症放射生物学原理 ;且属低能辐射 ,易防护。可有效补充改良根治术的清扫盲区。对控制乳腺癌局部复发近期效果明显 ,远期效果有待进一步观察 Objective To study the short-term curative effect of modified radical mastectomy combined with iodine-125 particle brachytherapy on breast cancer. Methods From June 2002 to November 2003, 26 patients with breast cancer underwent modified radical mastectomy. After parathyroidectomy, iodine-125 particles were placed along the longitudinal order of the internal thoracic artery , And Iodine - 125 particles were placed on the top and bottom of the clavicle. The incision and wound healing were observed after operation. The chest radiographs, blood and liver and kidney functions were reviewed. The local recurrence and metastasis were examined by systematic examination. Results were followed up for 3 to 20 months (mean October). No complications associated with implantation of iodine-125 were found. To date, no local recurrence has been found and 2 patients have distant metastases. Postoperative systemic response is small, no white blood cells, platelets decreased, no liver and kidney dysfunction. Conclusion Intraoperative iodine 125 particles have no adverse effect on postoperative incision healing of breast cancer. Targeted quasi-systemic reactions are mild and can be adjuvant chemotherapy such as chemotherapy immediately after surgery. Radiation therapy is more consistent with cancer radiobiology Principle; and is a low-energy radiation, easy protection. Can effectively supplement the modified radical mastectomy blind spots. The recent effect of local recurrence of breast cancer control effect is obvious, the long-term effect needs further observation
