目的探讨早期乳腺癌保乳手术联合术中放疗的临床疗效。方法 120例早期乳腺癌患者随机分为观察组与对照组,每组各60例,观察组患者采用手术+术中放疗治疗,对照组患者给予手术+术后常规放疗治疗,观察两组治疗效果。结果观察组患者切口愈合时间显著长于对照组(P<0.01),观察组术后切口水肿、脂肪液化等并发症的发生率显著高于对照组(P<0.01);术后1年乳房美容效果比较,观察组患者优良率为85.0%,显著高于对照组的63.3%(P<0.01);随访过程中,观察组患者局部复发2例(3.3%),死亡1例(1.7%);对照组患者复发1例(1.7%),无死亡病例,两组复发率及死亡率比较,差异无统计学意义(P>0.05)。结论手术+术中放疗在早期乳腺癌患者中具有良好效果,同时避免了全乳放疗导致的局部第二原发肿瘤,且可获得更好的美容效果,但临床应用中需注意对术后切口愈合的影响。
Objective To investigate the clinical efficacy of breast-conserving surgery combined with intraoperative radiotherapy in early breast cancer. Methods One hundred and twenty patients with early stage breast cancer were randomly divided into observation group and control group, 60 cases in each group. The patients in observation group were treated with operation and intraoperative radiotherapy. The patients in control group were given operation plus conventional radiotherapy. The therapeutic effect . Results The wound healing time in the observation group was significantly longer than that in the control group (P <0.01). The incidences of postoperative incision edema and fat liquefaction in the observation group were significantly higher than those in the control group (P <0.01) Compared with the control group, the excellent and good rate was 85.0% in the observation group and 63.3% in the control group (P <0.01). During the follow-up period, the local recurrence in the observation group was 2 cases (3.3%) and the death was 1 case (1.7% There was no recurrence in 1 patient (1.7%) in the group of patients. There was no significant difference in the recurrence rate and mortality among the two groups (P> 0.05). Conclusions Surgery and intraoperative radiotherapy have a good effect in early breast cancer patients, while avoiding the local second primary tumor caused by whole milk radiotherapy, and can achieve better cosmetic results, but clinical application should pay attention to the postoperative incision Healing effects.
