实质性和乳头状胰腺肿瘤很罕见,多见于年轻女性,仅侵犯局部,很少转移,肿瘤有时生长很大,但多数可切除,故应积极争取,可获长期生存率。作者报道6例的平均年龄为26岁,无临床征象2例,发现腹块4例。肿瘤位于胰头2例、胰体2例、胰尾1例和胰头、体、颈部1例。影像学检查发现胰肿瘤4例、钙化灶1例和钙化囊性肿瘤1例。均予手术切除,计胰十二指肠切除1例、局部切除2例、胰尾切除和胰空肠吻合2例、囊肿空肠吻合1例。病理均证实为乳头状胰腺实质性肿瘤,其中1例肿瘤很大,部分呈囊性变。随访观察,4例已生存40~74月。1例胰十二指肠切除病人发生糖尿病,术后66月死亡,与手术无
Substantial and papillary pancreatic tumors are rare and more common in young women. They only invade parts of the body and rarely metastasize. Tumors sometimes grow very large, but most are resectable, so they should be actively pursued to achieve long-term survival. The authors reported that the average age of 6 cases was 26 years old, no clinical signs were found in 2 cases, and 4 cases of abdominal masses were found. The tumor was located in 2 cases of pancreatic head, 2 cases of pancreatic body, 1 case of pancreas tail and 1 case of pancreatic head, body and neck. Imaging examination revealed 4 cases of pancreatic tumor, 1 case of calcification, and 1 case of calcified cystic tumor. Surgical resection was performed, including 1 case of pancreaticoduodenectomy, 2 cases of local resection, 2 cases of pancreatic jejunostomy and pancreaticojejunostomy, and 1 case of cyst jejunostomy. Pathology was confirmed as a substantial tumor of the papillary pancreas, including 1 case of large tumors, some of which were cystic changes. Follow-up observations, 4 cases have survived 40 to 74 months. One patient with pancreatoduodenectomy had diabetes and died 66 months after surgery.
