目的：利用医院-社区-家庭一体化合作管理模式对糖尿病患者进行管理，并进行效果评价。方法将76例糖尿病患者进行分组管理，比较实验组和对照组之间差异。结果实验组患者干预前后空腹血糖下降有显著性意义(P<0.05)，对照组的空腹血糖、甘油三脂、总胆固醇水平均呈上升趋势，具有显著性(P<0.05)。结论一体化管理模式促进了糖尿病患者相关知识、态度、行为的正向改变，有效控制了糖尿病患者的病情。“,”Objective:To use the hospital-Community-Family Integrated cooperative management model for diabetics to manage and conduct Evaluation.
