目的观察不同剂量的糖皮质激素治疗过敏性休克的临床效果。方法选取2013年1月-2014年12月医院收治的过敏性休克患者78例,随机分为低剂量组、中剂量组及高剂量各26例。3组均在常规治疗基础上加用氢化可的松治疗,剂量分为100mg(低)、200mg(中)、300mg(高)。观察3组患者的临床疗效、症状缓解时间、用药后的不良反应。结果高剂量组的临床有效率为96.15%明显高于中、低剂量组88.46%,差异有统计学意义(P<0.05);而中剂量组的临床有效率为92.31%显著高于低剂量组,差异有统计学意义(P<0.05)。高剂量组症状缓解时间显著少于中、低剂量组,差异有统计学意义(P<0.05);中剂量组症状缓解时间较低剂量组短,差异有统计学意义(P<0.05)。高、中剂量组不良反应率与低剂量组2组比较差异无统计学意义(P>0.05)。结论大剂量氢化可的松治疗过敏性休克,可快速缓解患者临床症状、缩短患者病程、快速纠正休克且不良反应少。
Objective To observe the clinical effect of different dosages of glucocorticoid on anaphylactic shock. Methods Seventy-eight patients with anaphylactic shock admitted from January 2013 to December 2014 in our hospital were randomly divided into low dose group, medium dose group and high dose of 26 patients. The three groups were treated with hydrocortisone on the basis of conventional treatment, the dose was divided into 100mg (low), 200mg (medium), 300mg (high). Three groups of patients were observed for clinical efficacy, symptom relief time, adverse reactions after treatment. Results The clinical effective rate of high dose group was 96.15%, which was significantly higher than that of middle and low dose group (88.46%), the difference was statistically significant (P <0.05); while the effective rate of middle dose group was 92.31% , The difference was statistically significant (P <0.05). The symptom relief time of high dose group was significantly less than that of middle and low dose group, the difference was statistically significant (P <0.05); middle dose group of symptom relief time shorter than the lower dose group, the difference was statistically significant (P <0.05). There was no significant difference in adverse reaction rate between high dose and middle dose group and low dose group (P> 0.05). Conclusion High-dose hydrocortisone in the treatment of anaphylactic shock can quickly relieve clinical symptoms, shorten the course of patients, quickly correct shock and less adverse reactions.
