目的:探讨PTEN、HER-2在子宫内膜样腺癌组织中的表达及意义。方法:运用免疫组织化学技术检测20例增生期子宫内膜、30例子宫内膜增殖症及60例子宫内膜样腺癌组织中PTEN、HER-2的表达。结果:PTEN在增生期子宫内膜、子宫内膜增殖症、子宫内膜样腺癌组织中阳性表达逐渐降低,并且在子宫内膜样腺癌组织中的表达与增生期子宫内膜、子宫内膜增殖症的表达比较差异有统计学意义(P<0.05);在子宫内膜样腺癌组织中PTEN的表达与病理组织学分级、临床分期有关(P<0.05),与肌层浸润及淋巴结转移无关(P>0.05)。HER-2在增生期子宫内膜、子宫内膜增殖症、子宫内膜样腺癌组织中阳性表达呈逐渐增高的趋势,HER-2在子宫内膜样腺癌中的表达与增生期子宫内膜表达比较差异有统计学意义(P<0.05),与子宫内膜增殖症比较差异无统计学意义(P>0.05),将增生期子宫内膜和子宫内膜增殖症合并为一组与子宫内膜样腺癌组比较差异有统计学意义(P<0.05);HER-2在子宫内膜样腺癌中有不同程度的表达,并随着肿瘤病理分级(P<0.01)、肌层浸润深度(P<0.01)、临床分期(P<0.01)的增加及出现淋巴结转移(P<0.05)时表达增强,且差异有统计学意义;子宫内膜样腺癌中PTEN表达与HER-2表达成负相关关系(rs=-1.000,P=0,α=0.01)。结论:PTEN的检测可作为子宫内膜样腺癌早期诊断的标记物,HER-2的检测可作为判断子宫内膜样腺癌预后的指征,PTEN的缺失和HER-2的过表达在子宫内膜样腺癌的发生、发展中具有一定意义。
Objective: To investigate the expression of PTEN and HER-2 in endometrioid adenocarcinoma and its significance. Methods: Immunohistochemical technique was used to detect the expression of PTEN and HER-2 in 20 cases of proliferative endometrium, 30 cases of endometrial hyperplasia and 60 cases of endometrial adenocarcinoma. Results: The positive expression of PTEN in proliferative endometrium, endometrial hyperplasia and endometrioid adenocarcinoma was gradually decreased, and the expression of PTEN in proliferative endometrium, uterus and endometrium The expression of PTEN in endometrioid adenocarcinoma was correlated with the histological grade and clinical stage (P <0.05), but not with myometrial invasion and lymph node No correlation with metastasis (P> 0.05). The expression of HER-2 in endometrioid adenocarcinoma was correlated with the proliferative phase in utero (P <0.05), but there was no significant difference with endometrial hyperplasia (P> 0.05). The hyperplasia endometrium and endometrial hyperplasia were combined into one group with uterus (P <0.05). The expression of HER-2 in endometrioid adenocarcinoma was different, and with the tumor pathological grade (P <0.01), myometrial invasion The expression of PTEN in endometriotic adenocarcinoma was significantly higher than that in HER-2 (P <0.01), clinical stage (P <0.01) and lymph node metastasis (P <0.05), and the difference was statistically significant A negative correlation was found (rs = -1.000, P = 0, α = 0.01). Conclusions: The detection of PTEN can be used as a marker for early diagnosis of endometrial adenocarcinoma. The detection of HER-2 can be used as an indication of the prognosis of endometrioid adenocarcinoma.
