目的了解中国内地社区获得性胃肠炎患者是否存在香港海鸥形菌感染。方法于2005年8月对杭州市第一人民医院肠道门诊就医患者采集粪样,分别接种SS、XLD、CCDA、TCBS、MacConkey琼脂(MA)以及改良头孢哌酮MacConkey琼脂((2MA)等6种平皿,对分离到的可疑细菌进行常规生理生化实验、16S rRNA基因序列测定、透射电镜(TEM)下形态观察及药敏试验。结果编号为242的标本,在CMA平皿上生长出无色、透明的微小菌落。该菌为革兰阴性,氧化酶、过氧化氢酶、尿素酶和精氨酸双脱氢酶阳性,鸟氨酸脱羧酶和赖氨酸脱羧酶阴性,不发酵葡萄糖。经16S rRNA基因序列测定,菌株242的基因序列长度为1413 bp,采用:Blast软件对测序结果进行同源性分析显示,与文献报道的香港海鸥形菌同源值达100%。TEM下可见菌体两端有多根鞭毛。药敏试验结果显示,该菌对临床常用的21种抗菌药物表现出不同程度的耐药性,其中对青霉素、氨苄西林、头孢哌酮、头孢他啶表现为耐药。结论证实中国内地社区获得性胃肠炎患者粪便中存在香港海鸥形菌。有关该菌引起感染性腹泻疾病的致病地位与流行病学等特征需进一步深入研究。
Objective To understand whether there is a Hong Kong seagull infection in community-acquired gastroenteritis patients in Mainland China. Methods In August 2005, fecal samples were collected from patients in the Intestinal Clinic of Hangzhou First People’s Hospital. SS, XLD, CCDA, TCBS, MacConkey agar (MA) and modified cefoperazone MacConkey agar (2MA) And then the suspicious bacteria were isolated and subjected to routine physiological and biochemical experiments, 16S rRNA gene sequence analysis, TEM observation and drug susceptibility test results.Results No.242 specimens were grown in CMA plates with no color, Transparent micro-colonies. The bacteria are Gram-negative, oxidase, catalase, urease and arginine bis-dehydrogenase-positive, ornithine decarboxylase and lysine decarboxylase negative, non-fermented glucose. The sequence of 16S rRNA gene was sequenced.The sequence of strain 242 was 1413 bp in length.The homology analysis of Blast software showed that the homology was 100% with that reported in the literature in Hong Kong.The TEM There were many flagella on both ends.The results of drug susceptibility test showed that the bacteria showed different degrees of resistance to the 21 kinds of antibacterial drugs commonly used clinically, of which penicillin, ampicillin, cefoperazone and ceftazidime showed resistance. It is confirmed that there is Hong Kong gull in Hong Kong in the feces of patients with community-acquired gastroenteritis in mainland China, and the pathogenicity and epidemiology of the infectious diarrhea caused by the bacteria need to be further studied.
