本文对30例早期肺心病(A组)44例甲状腺机能亢进症(B组)和20例正常人(C组)描记了肺阻抗图及其微分图。A与B组对比,肺阻抗图及其微分图九项参数中有七项有显著性差异。即Hs(Ω,p<0.01)HD(Ω,p<0.01),Q—B间期(sec,p<0.01)B—Y间期(sec,p<0.05),B—Y指数(p<0.01),Q—B/B—Y比值(p<0,01),dz/dt max(Ω/sec,p<0.01)。初步认为A与B组两者的鉴别点为:(1)甲亢时Hs、dz/dt max均值较正常组增高,早期肺心病相反降低,(2)甲亢时HD均值接近正常组,早期肺心病组较正常组显著降低;(3)甲亢时Q—B间期均值较正常组显著缩短,早期肺心病组相反延长;Q—B指数均值在A和B两组校正常组均延长;(4)甲亢时B—Y间期均值较正常组显著缩短、B—Y指数显著增大,但B—Y间期和B—Y指数均值早期肺心病组均较正常组缩短和减少。从而提示肺阻抗图及其微分图可做为早期肺心病和甲亢的鉴别诊断方法之一。
In this paper, 44 patients with hyperthyroidism (group B) and 20 normal subjects (group C) were enrolled in this study. Pulmonary impedance maps and their differential images were recorded in 30 patients with early stage pulmonary heart disease (group A). Compared with group B, there are significant differences in seven of the nine parameters of lung impedance map and its differential graph. (Hs) (p <0.01), B-Y interval (sec, p <0.01) and B-Y index 0.01), Q-B / B-Y ratio (p <0,01), dz / dt max (Ω / sec, p <0.01). The difference between A and B group was preliminarily considered as follows: (1) The mean value of Hs and dz / dt max in hyperthyroidism was higher than that in the normal group and concomitantly lower in the early stage of pulmonary heart disease; (2) The average HD value of hyperthyroidism was close to normal group, (3) In hyperthyroidism, the mean of Q-B interval was significantly shorter than that of the normal group and prolonged in the early stage of pulmonary heart disease group. The average of Q-B index was prolonged in both groups A and B; (4) ) Mean B-Y interval was significantly shorter and B-Y index was significantly increased in patients with hyperthyroidism than those in the normal group. However, the mean values of B-Y index and B-Y index in the early stage of pulmonary heart disease group were shorter and lower than those in the normal group. Thus suggesting that the lung impedance map and its differential map can be used as one of the differential diagnosis of early pulmonary heart disease and hyperthyroidism.
