目的分析并探讨二维超声联合四维超声对产前胎儿畸形的临床诊断价值。方法选择2011年6月至2014年7月在我院进行产前筛查的2500例中晚期孕妇为研究对象,对疑似存在胎儿畸形的孕妇给予单独二维超声检查和二维超声联合应用四维超声检查,根据孕妇引产结果或胎儿出生后情况判断其诊断准确性。结果 2500例中晚期孕妇共分娩2517个胎儿,共存在胎儿畸形126处,单纯二维超声检出胎儿畸形92处,诊断准确率为73.02%,联合检查检出胎儿畸形113处,诊断准确率为89.68%。联合检查诊断准确率显著高于单纯二维超声检查,差异具有统计学意义(P<0.05)。单纯二维超声检出胎儿心脏畸形17处,诊断准确率为77.27%,联合检查检出胎儿畸形21处,诊断准确率为95.45%。联合检查对胎儿心脏畸形诊断准确率显著高于单纯二维超声检查,差异具有统计学意义(P<0.05)。结论二维超声联合四维超声能显著提高产前胎儿畸形的诊断率,对降低出生缺陷、实现优生优育具有重要的临床意义。
Objective To analyze and discuss the value of two-dimensional ultrasound combined with four-dimensional ultrasound in the diagnosis of prenatal fetal malformations. Methods From June 2011 to July 2014 in our hospital for prenatal screening of 2,500 cases of pregnant women as the research object of suspected fetal malformations of pregnant women given a separate two-dimensional ultrasound and two-dimensional ultrasound combined four-dimensional ultrasound Check, according to the results of maternal labor or fetus after birth to determine the diagnostic accuracy. Results A total of 2517 fetuses were delivered in 2,500 middle-late pregnant women, with a total of 126 fetal malformations. Totally 92 fetal malformations were detected by simple two-dimensional ultrasonography. The diagnostic accuracy rate was 73.02%. The combined detection of fetal malformations was 113. The diagnostic accuracy was 89.68%. The diagnostic accuracy of joint examination was significantly higher than that of simple two-dimensional ultrasound, the difference was statistically significant (P <0.05). Simple two-dimensional ultrasound detected fetal heart malformation 17, the diagnostic accuracy was 77.27%, 21 cases of fetal malformations detected by joint examination, the diagnostic accuracy was 95.45%. The diagnostic accuracy of joint examination of fetal heart malformations was significantly higher than that of simple two-dimensional ultrasound, the difference was statistically significant (P <0.05). Conclusion Two-dimensional ultrasound combined with four-dimensional ultrasound can significantly improve the diagnostic rate of prenatal fetal malformations, which has important clinical significance in reducing birth defects and achieving prenatal and postnatal care.
