目的探讨对非增殖性糖尿病视网膜病变患者采用注射用丹参多酚酸盐联合羟苯磺酸钙胶囊治疗的临床疗效。方法将2014年4月至2015年12月68例非增殖性糖尿病视网膜病变患者按照随机数字表法分为对照组与观察组,各34例。两组患者均给予降糖、降压、降脂药物治疗;对照组加用羟苯磺酸钙胶囊治疗,500 mg/次,3次/d;观察组于对照组治疗基础上采用丹参多酚酸盐200 mg+生理盐水250 ml,1次/d。两组均治疗6个月。检测两组患者治疗前、后血糖及糖化血红蛋白水平、视力、血液流变学、视网膜黄斑区平均厚度变化;记录治疗期间不良反应。采用眼底照相和眼底荧光血管造影对眼底情况进行观察并判断疗效。结果治疗6个月后,两组患者空腹血糖、餐后2 h血糖、糖化血红蛋白水平均明显低于治疗前(P均<0.05),且治疗后观察组餐后2 h血糖明显低于对照组(P<0.05)。治疗6个月后,观察组患者视力较治疗前明显提高(P<0.05),两组视网膜黄斑区平均厚度明显低于治疗前(P均<0.05)。血液流变学各项指标[除对照组全血黏度(中切)和观察组血浆黏度(高切)外]在两组患者治疗后均较治疗前明显下降(P均<0.05);两组血沉水平比较有统计学差异(P<0.05)。观察组治疗总有效率明显高于对照组(91.18%vs 58.82%,P<0.01)。治疗期间两组均无明显不良反应发生。结论应用注射用丹参多酚酸盐联合羟苯磺酸钙胶囊治疗非增殖性糖尿病视网膜病变,疗效优于单用羟苯磺酸钙胶囊治疗,且安全性高。
Objective To investigate the clinical efficacy of Salviae miltiorrhizae polyphenols combined with calcium dobesilate in patients with non-proliferative diabetic retinopathy. Methods From April 2014 to December 2015, 68 cases of non-proliferative diabetic retinopathy were divided into control group and observation group according to random number table method, 34 cases in each group. Both groups were given hypoglycemic, antihypertensive, lipid-lowering drug treatment; control group with calcium dobesilate 500 mg / time, 3 times / d; the observation group in the control group based on the treatment of Salvia miltiorrhiza polyphenols Acid 200 mg + saline 250 ml, 1 time / d. Both groups were treated for 6 months. The blood glucose, HbA1c, visual acuity, hemorheology and the average thickness of the macular area were measured before and after treatment. The adverse reactions were recorded during the treatment. The use of fundus photography and fundus fluorescein angiography to observe the situation and determine the efficacy of the fundus. Results After 6 months of treatment, the levels of fasting blood glucose, postprandial 2-hour postprandial blood glucose and glycosylated hemoglobin were significantly lower in both groups (P <0.05), and the postprandial blood glucose in observation group 2 h after treatment was significantly lower than that in the control group (P <0.05). After 6 months of treatment, the visual acuity in the observation group was significantly higher than that before treatment (P <0.05). The average thickness of the macular area in the two groups was significantly lower than that before treatment (all P <0.05). The indexes of hemorheology [except the whole blood viscosity of the control group (middle-cut) and the plasma viscosity of the observation group (high-cut) were significantly lower than those before treatment in both groups after treatment (P all <0.05) Sputum levels were statistically significant differences (P <0.05). The total effective rate of observation group was significantly higher than that of control group (91.18% vs 58.82%, P <0.01). No significant adverse reactions occurred in both groups during treatment. Conclusion Salviae miltiorrhizae polyphenols and calcium dobesilate capsules for the treatment of non-proliferative diabetic retinopathy, the effect is better than single calcium dobesilate treatment, and high safety.
