患者,男,2岁,住院号:44024.左眼角流脓3个月.1992年1月初,左眼颞侧红肿痛,并发烧,破溃流脓,无盗汗.在当地给注射青、链霉素治疗,局部换药,伤口很快愈合.不久外眼角又突出一脓头,并破溃流脓.虽抗炎换药总不愈合.检查:左眼眶外壁柔软,波动,压之脓液从外眦破溃处流出,有恶臭味.外眼角内陷.眼球运动不受限.瞳孔光反射正常,等大.眼前段及眼底正常.脓液培养为大肠埃希氏菌.对青霉素,氨苄青霉素,羧苄青霉素、庆大霉素、洁霉素耐药,对痢特灵敏感.CT(00484号)检查:左眼眶中上部外上方,可见一椭圆形密度阴影,部份向外壁突出,CT值15—22Hz,外围有较高密度的阴影.眶外壁骨质破坏,破坏向上达额骨,并有点状骨影.眼肌、视神经无明显异常.于1992年4月15日收治.
