目的探讨视神经管骨折高分辨率CT(HRCT)检查的影像特征及其临床应用价值。方法对22例视神经管骨折患者行层厚为1.5mm的视神经管轴位及冠状位HRCT扫描,并对视神经管骨折的CT表现进行分析。结果视神经管骨折的直接征象表现为视神经管壁骨质连续性中断,可分为凹陷型(27.3%)、线状型(22.7%)、粉碎型(27.3%)、嵌入型(9.1%)、混合型(13.6%)5型。视神经管骨折的间接征象表现为蝶窦积血(95.5%)、筛窦积血(50.0%)、视神经增粗(36.4%)。22只眼中,视神经管内侧壁骨折15只眼,占68.2%;外侧壁骨折1只眼,占4.5%;上壁骨折1只眼,占4.5%;下壁骨折2只眼,占9.1%;多发骨折3只眼,占13.6%。结论视神经管HRCT检查可清楚的显示视神经管骨折的部位并对其分型,可为临床诊断及选择治疗方案提供可靠的影像学依据。
Objective To investigate the imaging characteristics of high resolution CT (HRCT) examination of optic canal fracture and its clinical value. Methods Twenty-two patients with optic nerve canal fracture underwent 1.5-mm HRCT scans of the optic canal and coronal planes. The CT findings of optic canal fracture were analyzed. Results The direct signs of optic canal fracture showed discontinuity of the continuity of the optic nerve wall, which could be divided into concave type (27.3%), linear type (22.7%), shredded type (27.3%), embedded type Mixed (13.6%) Type 5 Indirect signs of optic canal fracture showed splenic hemorrhage (95.5%), ethmoid hemorrhage (50.0%), optic nerve thickening (36.4%). Among the 22 eyes, 15 eyes of the inner wall of the optic canal accounted for 68.2%, 1 of the lateral wall fractures accounted for 4.5%, 1 of the upper wall fractures accounted for 4.5%, 2 of the lower wall fractures accounted for 9.1% Multiple fractures in 3 eyes, accounting for 13.6%. Conclusion HRCT examination of optic canal can clearly show the location of the optic canal fracture and classify it, which can provide a reliable imaging basis for clinical diagnosis and treatment options.
