报告纤维组织细胞肉瘤32例,男19例,女13例,年龄18~81岁,80%在40岁以上。肿瘤最大直径1.7~33cm,平均10cm。镜下呈多形性,主要由组织细胞群样细胞和纤维母细胞样细胞构成。在二者之间有过渡型细胞。由组织细胞样细胞,可演变为泡沫状细胞和多核及单核瘤巨细胞。另外见极少量未分化间叶细胞。瘤细胞有不同程度的异型,典型和不典型核分裂象多见于组织细胞样细胞。瘤细胞呈三种排列方式:花瓣状、束状和多形性结构。本组32例分为组织细胞样细胞为主型(4例)、纤维母细胞样细胞为主型(13例)、多形性(5例)和混合型(10例)。在花瓣状结构的中央,为模糊的脉管,其外紧贴着未分化间叶细胞,外为其它各种瘤细胞。这些细胞之间,见连续“光谱”样过渡,因此,支持该瘤由未分化间叶细胞起源。这种未分化间叶细胞向组织细胞样细胞和纤维母细胞样细胞分化。组织细胞又可演变为泡沫状细胞和单核及多核瘤巨细胞,从而造成纤维组织细胞肉瘤的多形性。
Reported 32 cases of fibrous tissue sarcoma, 19 males and 13 females, aged 18 to 81 years old, 80% over 40 years of age. The maximum diameter of the tumor is 1.7 to 33 cm with an average of 10 cm. Microscopically, it is pleomorphic, consisting mainly of histiocytes and fibroblast-like cells. There are transitional cells between the two. From histiocyte-like cells, it can evolve into foamy cells and multinuclear and monocyte giant cells. Also see a very small amount of undifferentiated mesenchymal cells. Tumor cells have different degrees of heterotypic, typical and atypical mitoses are more common in histiocyte-like cells. Tumor cells are arranged in three ways: petal, bundle and polymorphous structures. This group of 32 cases were divided into histologically cell-like cells (4 cases), fibroblast-like cells (13 cases), polymorphism (5 cases) and mixed type (10 cases). In the center of the petaloid structure, there are fuzzy vessels, outside of which there are undifferentiated mesenchymal cells and outside are various other tumor cells. Between these cells, a continuous “spectrum”-like transition is seen, and therefore, the tumor is supported by undifferentiated mesenchymal cells. This undifferentiated mesenchymal cells differentiate into histiocyte-like cells and fibroblast-like cells. Tissue cells can in turn evolve into foamy cells and mononuclear and multinuclear giant cells, resulting in pleomorphic fibrous sarcoma.
