目的探讨椎动脉磁共振血管成像(MRA)、经颅多普勒(TCD)及椎动脉超声对椎基底动脉系统缺血性卒中的临床诊断价值。方法对41例椎基底动脉系统缺血性卒中的患者进行椎动脉 MRA、TCD 及椎动脉超声检测,观察其异常情况。结果41例患者中椎动脉 MRA 异常27例(65.85%),TCD 异常32例(78.05%),椎动脉超声异常25例(60.98%)。椎动脉 MRA 能清楚地显示血管病理形态,主要表现为椎动脉局部狭窄、单侧椎动脉变细及血流信号降低。结论椎动脉 MRA、TCD 与椎动脉超声的联合检查有助于椎基底动脉系统缺血性卒中的病因诊断。
Objective To investigate the clinical value of vertebral artery magnetic resonance angiography (MRA), transcranial Doppler (TCD) and vertebral artery ultrasound in the diagnosis of vertebrobasilar ischemic stroke. Methods 41 cases of vertebrobasilar ischemic stroke patients were examined by MRA, TCD and ultrasonic examination of vertebral artery. The abnormalities were observed. Results There were 27 cases (65.85%) with abnormal MRA in vertebral artery, 32 cases (78.05%) with abnormal TCD and 25 cases (60.98%) with abnormal ultrasound in vertebral artery. Vertebral artery MRA can clearly show the vascular pathology, mainly as localized vertebral artery stenosis, unilateral vertebral artery thinning and blood flow signal reduction. Conclusion The combined examination of MRA, TCD and ultrasound in vertebral artery can be helpful for the diagnosis of ischemic stroke in vertebrobasilar artery.
