目的探讨手足口病并发急性迟缓性麻痹的治疗方法。方法回顾性收集临床收治的HFMD合并AFP患儿47例,所有病例在常规治疗的基础上,根据是否应用糖皮质激素系统治疗将其分为二组。治疗组急性期给予甲强龙冲击治疗5 d后改口服强的松,总疗程2月。对照组仅急性期给予甲强龙冲击治疗3~5 d。结果二组在出院后3、6个月随访,查肢体肌力恢复情况。治疗组迟缓性麻痹肢体恢复率均高于对照组,χ2检验P均小于0.05,有统计学意义。结论应用糖皮质激素可有效地减轻了免疫病理过程等多种机制引起脊髓前脚细胞损伤,促进瘫痪肢体的功能恢复。
Objective To investigate the treatment of hand-foot-mouth disease complicated by acute delayed paralysis. Methods Forty-seven patients with HFMD complicated with AFP were enrolled in this retrospective study. All cases were divided into two groups according to whether they were treated with glucocorticoid or not. Acute treatment of patients given methylprednisolone treatment 5d after oral prednisone, the total course of treatment in February. The control group was treated with methylprednisolone only for 3 ~ 5 days in acute phase. Results The two groups were followed up 3 and 6 months after discharge to check the recovery of limb muscle strength. Treatment group delayed paralysis limb recovery rate were higher than the control group, χ2 test P less than 0.05, with statistical significance. Conclusion The application of glucocorticoids can effectively alleviate the immunopathological process and other mechanisms leading to injury of spinal cord anterior foot cells and promote functional recovery of paralyzed limbs.
