目的探讨老年男性冠状动脉粥样硬化性心脏病(CHD)患者其体内性激素水平的变化与血脂之间的关系。方法所有研究对象均检测雌二醇(E2)、睾酮(T)、血脂、射血分数(EF)水平,比较各组间上述指标的水平差异并进行相关分析。结果CHD心力衰竭组和非心力衰竭组间E2、T、E2/T水平差异无显著性(P>0.05),CHD组E2及T水平明显低于非CHD组(P<0.05),而E2/T在CHD组明显高于非CHD组(P<0.05);E2、T、E2/T水平与冠脉狭窄程度、冠脉病变支数、冠脉病变评分之间无相关性,而EF值及血脂水平则与之相关;CHD患者E2水平与HDL-C水平呈显著负相关,与TC、LDC-C、TG、LP(a)水平呈显著正相关,T与血脂间无显著相关性。结论老年男性CHD患者体内性激素水平明显下降,且以雄性激素下降为主;体内性激素水平与CHD的严重程度无关;体内雌激素水平与血脂代谢有明显相关,而雄激素变化与血脂代谢无明显相关。
Objective To investigate the relationship between the level of sex hormone in the elderly male patients with coronary atherosclerotic heart disease (CHD) and serum lipids. Methods All subjects were tested for estradiol (E2), testosterone (T), lipids, and ejection fraction (EF) levels. The differences among the above groups were compared and analyzed. Results There was no significant difference in the levels of E2, T and E2 / T between CHD heart failure group and non-heart failure group (P> 0.05). The levels of E2 and T in CHD group were significantly lower than those in non-CHD group (P <0.05) T in CHD group was significantly higher than that in non-CHD group (P <0.05). There was no correlation between the levels of E2, T and E2 / T and coronary artery stenosis, The levels of serum lipids were correlated with them. The levels of E2 in patients with CHD were significantly negatively correlated with the level of HDL-C, positively correlated with the levels of TC, LDC-C, TG and LP (a). There was no significant correlation between T and plasma lipids. Conclusion The levels of sex hormones in CHD patients were significantly decreased in male CHD patients, and the concentration of androgen decreased. The levels of sex hormones in vivo were not related to the severity of CHD. The levels of estrogen in serum were significantly correlated with the lipid metabolism, while the changes of androgen did not correlate with lipid metabolism .
