目的探讨甲状腺微小乳头状癌患者超声引导下经皮微波消融治疗与外科手术切除治疗的临床效果与安全性分析。方法将采取超声引导下细针穿刺活检(FNAB)确诊为甲状腺乳头状癌的65例患者,结节最大直径<1.0cm。根据患者自愿,消融组30例,手术组35例。观察两组的手术时间、出血、并发症、术后甲状腺相关激素变化及消融组术后肿瘤的体积缩小情况。结果消融组具有手术时间短、术中出血少、住院时间短、术后甲状腺相关激素波动小,差异均有统计学意义(P<0.05)。两组术后并发症及有效率无统计学意义(P>0.05)。结论超声引导下微波消融治疗甲状腺微小乳头状癌较手术有明显优势,为不愿手术及有手术禁忌证的患者的一种新型微创治疗方法。
Objective To investigate the clinical effect and safety of ultrasound-guided percutaneous microwave ablation and surgical resection in patients with thyroid papillary carcinoma. Methods Totally 65 cases of thyroid papillary carcinoma diagnosed by ultrasound-guided fine needle aspiration biopsy (FNAB) were selected. The maximum nodule diameter was less than 1.0 cm. According to the patient’s voluntary, ablation group of 30 cases, 35 cases of surgery group. The operation time, bleeding, complications, postoperative thyroid-related hormone changes and tumor size reduction after ablation group were observed. Results The ablation group had shorter operative time, less intraoperative bleeding, shorter hospital stay, and less postoperative thyroid-related hormone fluctuations. The difference was statistically significant (P <0.05). The postoperative complications and effective rate were not statistically significant (P> 0.05). Conclusion Ultrasound-guided microwave ablation of papillary thyroid carcinoma has obvious advantages over surgery. It is a new minimally invasive treatment for patients who are unwilling to undergo surgery and contraindications to surgery.
