目的探讨社区获得性肺炎及肺炎并脓毒症患儿血清转铁蛋白(TF)、D-二聚体(DD)水平及其临床意义。方法选择2008年1月-2009年12月本院收住的肺炎并脓毒症患儿29例(肺炎并脓毒症组)、社会获得性肺炎患儿25例(肺炎组)及本院体检健康儿童20例(健康对照组)。分别于入院5d内及体检时抽取空腹静脉血3mL,取上清液,检测其血清TF和DD水平。结果肺炎并脓毒症组TF水平[(1.63±0.86)g.L-1]明显低于肺炎组[(2.04±0.59)g.L-1]和健康对照组[(2.49±0.86)g.L-1](Pa<0.05),肺炎组显著低于健康对照组(P<0.05)。肺炎并脓毒症组血清DD水平明显高于肺炎组(P<0.05),肺炎组血清DD水平与健康对照组比较差异无统计学意义(P>0.05)。结论 TF、DD参与肺炎并脓毒症的病理生理过程,检测TF和DD对病情判断及评估预后有一定参考意义。
Objective To investigate the serum levels of transferrin (TF) and D-dimer (DD) in children with community-acquired pneumonia and pneumonia and sepsis and their clinical significance. Methods From January 2008 to December 2009, 29 children with pneumonia and sepsis admitted to our hospital (pneumonia and sepsis), 25 children with pneumonia (pneumonia) and 25 children with acquired pneumonia 20 healthy children (healthy control group). Fasting venous blood 3 mL was drawn within 5 days after admission and during physical examination. The supernatants were taken for determination of serum TF and DD levels. Results The level of TF in pneumonia and sepsis group was significantly lower than that in control group [(1.63 ± 0.86) gL-1] [(2.04 ± 0.59) gL-1] and healthy control group [(2.49 ± 0.86) gL-1] <0.05), pneumonia group was significantly lower than the healthy control group (P <0.05). Serum DD level in pneumonia and sepsis group was significantly higher than that in pneumonia group (P <0.05). There was no significant difference in serum DD level between pneumonia group and healthy control group (P> 0.05). Conclusions TF and DD are involved in the pathophysiological process of pneumonia and sepsis. The detection of TF and DD may be of some reference value for judging the prognosis and prognosis.
