目的:探讨严重眼球破裂损伤玻璃体视网膜手术挽救视力的价值。方法:回顾性选取2012年1月至2015年1月我院收治的60例严重眼球破裂伤患者为研究对象,均给予玻璃体视网膜手术救治,观察患者的手术治疗效果。结果:经手术治疗,60例患者的眼球均得以保存;术后视网膜复位情况:手术后行眼底检查,其中视网膜平复43例,视网膜复位不佳13例,4例未能窥清眼底;术后最佳视力矫正:与治疗前相比,术后无光感眼6例,光感眼7例,眼前手动20眼,眼前指数12眼;术后随访3个月以后,无光感眼6例,光感眼7例,眼前手动18眼,眼前指数14眼, 41例患者的视力明显增高,15例患者的视力未得到明显改善;术后并发症:术后2眼有硅油进入前房,给予俯卧体位处理后,1眼房硅油留存,患者无任何光感;术后12例患者前房有积血,术后3个月随访均吸收,1眼内眼压升高,给予药物治疗后恢复正常,随访24个月,1例患者眼球轻度萎缩。结论:严重眼球破裂损伤行玻璃体视网膜手术后能够在一定程度上恢复视力,保留眼球形态,同时,还可降低并发症的发生率,值得临床推广使用。
Objective: To investigate the value of vitreoretinal surgery to save eyesight after severe ocular rupture. Methods: Sixty patients with severe ocular rupture admitted in our hospital from January 2012 to January 2015 were retrospectively selected for vitreo-retinal surgery and the surgical treatment effect was observed. Results: The eyes of 60 patients were preserved after surgery. The retinal reattachment was performed after fundus examination. Among them, 43 cases were retina retreated, 13 cases retreated poorly, and 4 cases failed to see the fundus. After operation The best visual acuity: compared with before treatment, 6 cases of blind sensory eye, 7 cases of sensory eye, 20 eyes before hand, anterior index of 12 eyes; 3 months after surgery, 6 cases of sensory eye without light , Eyes with light perception in 7 eyes, hands in front of eyes with 18 eyes in advance and eyes index in 14 eyes. Visual acuity of 41 patients was significantly increased, and visual acuity of 15 patients was not significantly improved. Postoperative complications: 2 eyes had silicone oil entering the anterior chamber, After the body position was treated in prone position, the silicone oil in 1-room was stored without any light sensation. After operation, 12 patients had hemorrhage in the anterior chamber and were followed up for 3 months. The intraocular pressure in 1 eye was increased. Returned to normal, followed up for 24 months, 1 patient mild eye atrophy. Conclusion: Severe rupture of eyeball can restore visual acuity to a certain extent after vitreoretinal surgery and preserve the shape of eyeball. At the same time, it can reduce the incidence of complications and is worthy of clinical application.
