目的:通过对缺牙区骨量不足患者实施骨环移植种植术,评价骨环植骨同期植入种植体的临床效果,讨论其应用价值。临床资料及方法:8例患者通过自体骨移植术取骨(下颌骨颏部3例,下颌磨牙后区5例),制备骨环植入并同期植入Ankylos种植体10枚,前牙2枚,前磨牙3枚,磨牙5枚,缺隙区可用自体碎骨或骨再生材料填补。龈下愈合,在这个阶段软组织要无刺激,并定期复诊。6-9个月后,经临床检查和影像学摄片评价骨环技术在改善缺牙区骨量不足时种植的临床效果。结果:8例患者植入10枚种植体均无松动脱落情况,取骨区术后愈合良好,X线片示受植区种植体骨结合状况佳。7例患者受植区预后良好,牙槽嵴高度和厚度均达到理想水平。1例患者受植区骨环顶部分暴露(种植体未暴露),附着龈退缩,X线片无明显异常。经治疗症状已明显改善。结论:骨环技术为缺牙区三维骨量不足时即刻种植提供可能,并有效保证了缺牙区牙槽嵴三维骨量的恢复。与经典的二阶段骨增量种植术相比,骨环技术减少了种植治疗步骤,减少了植骨愈合期骨量损失,临床效果佳。不过需要注意对适应症的选取。
OBJECTIVE: To evaluate the clinical effect of implanting bone graft during the same period by implanting the bone graft in patients with bone loss in the edentulous area, and to discuss its application value. Clinical data and methods: Eight patients underwent bone autografting (3 cases of mandibular chin and 5 cases of posterior mandibular molars). Bone rings were implanted and implanted with 10 Ankylos implants and 2 anterior teeth , 3 premolar teeth, molars 5, the gap can be filled with bone or bone regeneration material. Subgingival healing, soft tissue at this stage to be no stimulation, and regular referral. 6-9 months after clinical examination and imaging radiography evaluation of bone ring technology in improving the lack of bone loss in the lack of planting the clinical effect. Results: There was no loosening and dropping of the 10 implants in 8 patients, and the healing of the bone graft area was good. The X-ray showed that the implants in the implants were well bonded. The prognosis was good in 7 patients, and the alveolar ridge height and thickness reached the ideal level. One patient was exposed to the top of the bone ring at the implant site (the implants were not exposed), and the gingival margin was retracted with no obvious abnormalities on the X-ray film. After treatment symptoms have been significantly improved. CONCLUSION: The bone ring technique offers the possibility of implanting immediately when the three-dimensional bone mass is insufficient in the edentulous area and effectively restoring the three-dimensional bone mass of the alveolar ridge in the edentulous area. Compared with the classic two-stage bone incremental implant surgery, the bone ring technique reduces the steps of planting treatment and reduces the bone loss during the healing of the bone graft, which has a good clinical effect. But need to pay attention to the selection of indications.
