目的探讨女性肛门尖锐湿疣(CA)组织中人乳头瘤病毒(HPV)感染的基因型及其临床意义。方法选取1985年1月~2015年5月江苏省苏州市吴江区中医院、南京市中医院、常州市中医院肛肠科手术切除的女性肛门及肛管CA石蜡组织标本150份,提取23种HPV DNA,采用基因扩增结合基因芯片技术对其组织进行23种HPV分型的检测,并对患者临床病理资料进行分析。结果 150例女性肛门CA组织标本中检出HPV阳性者111例,HPV感染率为74.00%(111/150)。其中单一型HPV感染72例,阳性检出率为48.00%(72/150),多型HPV感染39例,阳性检出率为26.00%(39/150)。单一型HPV感染中HPV11型34例,阳性检出率为22.67%(34/150),HPV11型是最主要的感染型别;HPV6型29例,阳性检出率为19.33%(29/150)。多型HPV感染中,HPV6+11型13例,占多型感染的33.33%(13/39),是多型感染的主要型别;HPV11+18型5例,占多型感染的12.82%(5/39)。结论 HPV6型、11型、6+11型、11+18型感染是女性肛门CA最主要的型别。基因扩增结合基因芯片技术是适合临床应用于HPV分型检测的诊断方法,其敏感性和特异性较好,适合分子流行病学的研究。
Objective To investigate the genotypes of human papillomavirus (HPV) infection in female genital warts (CA) and its clinical significance. Methods From January 1985 to May 2015, 150 female and anal CA paraffin specimens of female anus and anus were surgically removed from Wuchang District Hospital of Traditional Chinese Medicine, Nanjing Hospital of Traditional Chinese Medicine and Changzhou Hospital of Traditional Chinese Medicine, Suzhou City, Jiangsu Province. DNA, using gene amplification combined with gene chip technology to detect 23 kinds of HPV in their tissues, and analyze the clinical and pathological data of the patients. Results Among the 150 cases of female anal CA specimens, 111 cases were positive for HPV, the HPV infection rate was 74.00% (111/150). Among them, 72 were single-type HPV infection, the positive rate was 48.00% (72/150), and 39 were multi-type HPV infection. The positive rate was 26.00% (39/150). In the single type of HPV infection, 34 cases were HPV11, the positive rate was 22.67% (34/150). HPV11 was the most common infection type. In HPV6 type 29 cases, the positive rate was 19.33% (29/150) . In multi-type HPV infection, 13 cases were HPV6 + 11 type, accounting for 33.33% (13/39) of multi-type infection, which was the major type of multi-type infection. HPV11 + 18 type accounted for 12.82% 5/39). Conclusion HPV6, type 11, type 6 +11 and type 11 + 18 are the most predominant types of female anal CA. Gene amplification combined with gene chip technology is suitable for clinical diagnosis of HPV typing detection method, its sensitivity and specificity is good, suitable for molecular epidemiological studies.
