高压氧联合针灸辅助治疗对糖尿病周围神经病变患者临床疗效、神经功能及相关血清因子的影响

目的:探讨高压氧联合针灸辅助治疗对糖尿病周围神经病变(diabetic peripheral neuropathy, DPN)患者神经传导速度、腓肠肌神经阈值、病情相关因子及氧化应激的影响。方法:从2018年6月至2019年5月烟台山医院内分泌科收治的DPN患者中随机选取114例,按照随机数字表法分为研究组(57例)与对照组(57例)。2组患者均给予控制血糖、饮食疗法及运动指导等常规治疗,在此基础上,对照组仅给予高压氧治疗,研究组给予高压氧联合针灸治疗。观察2组患者的临床疗效、神经传导速度、腓肠肌神经阈值、病情相关因子和氧化应激指标变化。结果:对照组和研究组患者临床有效率分别为77.19%和91.23%,研究组明显高于对照组,差异有统计学意义(n P<0.05)。与治疗前相比,治疗后研究组患者胫神经、腓神经、正中神经的感觉神经传导速度和运动神经传导速度均明显快于对照组,腓肠肌神经阈值均明显低于对照组,血清C肽(C peptide,CP)、脑源性神经营养因子(brain derived neurotrophic factor,BDNF)、基质细胞衍生因子-1α(stromal cell derived factor -1α,SDF-1α)、超氧化物歧化酶(superoxide dismutase,SOD)、谷胱甘肽过氧化物酶(glutathione peroxidase , GSH-Px)、一氧化氮(nitric oxide,NO)、过氧化氢酶(catalase,CAT)水平均明显高于对照组,高迁移率族蛋白B1(high mobility group protein B1,HMGB1)、髓鞘碱性蛋白(myelin basic protein,MBP)、胱抑素C(cystatin C,CysC)及丙二醛(malondialdehyde ,MDA)水平均明显低于对照组,差异均有统计学意义(均n P<0.05)。n 结论:高压氧联合针灸治疗DPN的临床疗效显著,能够明显改善患者的神经传导速度,调节HMGB1、CP、BDNF、MBP、SDF-1α、CysC等因子水平,减轻机体氧化应激反应。“,”Objective:To investigate the efficacy of hyperbaric oxygen combined with acupuncture on nerve conduction velocity, sural nerve threshold, disease-related factors, and oxidative stress in patients with diabetic peripheral neuropathy (DPN).Methods:A total of 114 DPN patients, treated at the Department of Endocrinology of Yantai Mountain Hospital from June 2018 to May 2019, were randomly selected and divided into the study group (57 cases) and the control group (57 cases) by the random number table method. Patients in both groups were given routine treatments including blood glucose control, diet therapy, and exercises under guidance. On this basis, the control group was only given hyperbaric oxygen (HBO) treatment, while the study group was given HBO combined with acupuncture. The clinical efficacy and the indexes of nerve conduction velocity, sural nerve threshold, disease-related factors, and oxidative stress were observed.Results:The clinical efficacy of the control group was 77.19%, and that of the study group was 91.23%. The latter was significantly higher than the former (n P<0.05). Compared with the indexes before treatment, the sensory nerve conduction velocities and motor conduction velocities of tibial nerve, sural nerve, and median nerve in the study group were all significantly higher than those of the control group. The sural nerve threshold in the study group was obviously lower than that of the control group. The levels of sursum C peptide (CP), brain derived neurotrophic factor (BDNF), stromal cell derived factor-1α (SDF-1α), superoxide dismutase (SOD), glutathione peroxidase (GSH-Px), nitric oxide (NO), and catalase (CAT) in the study group were all obviously higher than those in the control group; while the levels of high mobility group protein B1 (HMGB1), myelin basic protein (MBP), cystatin C (CysC), and malondialdehyde (MDA) in the study group were all significantly lower than those in the control group. All these differences were statistically significant (n P<0.05).n Conclusion:HBO combined with acupuncture can achieve obvious therapeutic effects on DPN by significantly improving the nerve conduction velocity, regulating the levels of HMGB1, CP, BDNF, MBP, SDF-1α, CysC and other factors, and easing the oxidative stress response.
