目的研究新生儿静脉导管闭合时间及其Z值回归方程,探讨新生儿静脉导管闭合时间随胎龄的变化规律。方法应用彩色多普勒超声探测不同胎龄新生儿静脉导管闭合的具体时间,以胎龄为自变量(X)对所测的静脉导管闭合时间(Y)进行相关性回归分析,建立新生儿静脉导管闭合时间的Z值回归方程(ln Y=a+b X+cX~2),根据公式Z=(M-Y)/Sx(M为观察值,Y为预测平均值),计算出不同胎龄新生儿静脉导管闭合时间的Z值。结果共获得432例新生儿静脉导管闭合时间,其随胎龄增加而缩短(r=-0.938,P<0.001),胎龄31~41周新生儿静脉导管闭合时间参考值分别为28.5、26.3、23.7、20.9、18.0、15.2、12.5、10.1、7.9、6.2、4.6天。所获得非线性回归方程式为lnY=-5.228+0.089X-0.000228X~2,R~2=0.854,S_x=0.214(P<0.001),由各胎龄新生儿静脉导管闭合时间的预测平均值及S_x可计算静脉导管闭合时间任一测定值的Z值,Z=(M-Y)/S_x(其中M为观察值,Y为预测平均值);Z值呈正态分布,无随胎龄变化趋势。结论新生儿静脉导管闭合时间与胎龄存在相关性,采用非线性回归方程可用于计算预测平均值,所获得的Z值呈正态分布。
Objective To study the closure time of neonatal venous catheters and the regression equation of Z value, and to explore the change law of neonatal venous catheter closure time with gestational age. Methods Color Doppler ultrasound was used to detect the specific time of IVC closure in neonates of different gestational ages. Correlation analysis of the IV occlusion time (Y) with gestational age as independent variable (X) was performed to establish neonatal intravenous According to the equation Z = (MY) / Sx (M is the observation value and Y is the prediction average value), the regression equation of the Z value of the catheter closure time is calculated Z-value of the closure time of the pediatric catheter. Results A total of 432 neonate venous catheter closure time was obtained, which was shortened with the increase of gestational age (r = -0.938, P <0.001). The reference values of IVC closure time of neonates with gestational age of 31-41 weeks were 28.5,26.3, 23.7, 20.9, 18.0, 15.2, 12.5, 10.1, 7.9, 6.2, 4.6 days. The nonlinear regression equations were obtained by predicting the average value of the closure time of the venous catheters of newborn infants of each gestational age and the mean of lnY = -5.228 + 0.089X-0.000228X ~ 2, R ~ 2 = 0.854, S_x = 0.214 S_x can calculate the Z value of any measured value at the closure time of the venous catheter, Z = (MY) / S_x (where M is the observation value and Y is the predicted average value); the Z value is normal distribution with no change trend with gestational age. Conclusion Neonatal venous catheter closure time is correlated with gestational age. The nonlinear regression equation can be used to calculate the predicted average value, and the Z value obtained is normal distribution.
