目的:了解新生儿早期黄疸的发生情况,探讨干预的必要性。方法:采用经皮胆红素测定法监测母婴同室新生儿住院期间胆红素,同时对82例新生儿测定血清胆红素,以比较经皮胆红素与血胆红素之间的关系;记录有可能影响新生儿胆红素的围生因素;记录全部母亲的ABO血型;测定黄疸较显著的新生儿ABO血型51例。结果:发现皮肤黄染并测定经皮胆红素2 461例,早期新生儿黄疸发生率为76.5%,经皮胆红素平均峰值为245.5±25.2。≥256.5μmg/L的1 286例,占监测对象的40.0%。<24 h内出现黄疸者12例,占0.37%。早产、出生低体重、围生期缺氧对黄疸程度均有一定影响,其中出生时窒息的影响有显著性差异。O型血母亲所生A、B型血的新生儿黄疸程度高于其他新生儿,差异有显著性。结论:新生儿早期黄疸发生率较高,1/2以上的新生儿胆红素峰值高于传统的生理性范畴。绝大部分经过顺利、预后良好,无需特殊的处理。经皮测胆红素是一种简便有效的监测方法,有良好的参考意义。
Objective: To understand the incidence of neonatal jaundice early, to explore the necessity of intervention. Methods: The perinatal bilirubin assay was used to monitor the bilirubin during in-hospital neonatal maternal and neonatal period. Conclusion: The incidence of jaundice in early neonates is high, and the peak value of bilirubin in neonates with more than 1/2 is higher than the traditional physiological category. Most of the smooth, good prognosis, without special treatment. Percutaneous bilirubin is a simple and effective monitoring method, a good reference value.
