目的:寻找能够客观灵敏地反映脂肪肝血流动力学状态的综合性指标及参数。方法:在临床化验和B超形态学确诊脂肪肝的基础上,应用肝阻抗法,采用块状电极检测,按相应年龄段设健康对照组,统计分析用均数配对t检验。结果:随着脂肪肝由I组到Ⅲ组的进展,Hs(肝血流灌注波幅)、Cdz/dtmax(肝血流灌注速率)、C/B—C(肝血流灌注指数)、B—C(快速射血间期)、I环和总环均逐渐降低与缩小,而Hd/Hs(说明肝静脉、门静脉回流阻力)、Q—B/B—Y(肝血流灌注阻力)则逐渐增大,与相应健康组对比其差异有高度显著性(P<0.01)。结论:应用肝阻抗血流图Hs、Hd/Hs、肝阻抗微分图Cdz/dtmax、C/B—C、B—C、Q—B/B—Y及阻抗微分环中I环、总环,可以客观敏感地综合反映脂肪肝的血流动力学状态。
Objective: To find a comprehensive index and parameters that objectively and sensitively reflect the hemodynamic status of fatty liver. Methods: On the basis of clinical laboratory test and B-mode morphologic diagnosis of fatty liver, the liver impedance method was used, the block electrode test was used, and the healthy control group was set according to the corresponding age group. The statistical analysis was used to paired t-test. Results: With the progress of fatty liver from group I to group III, Hs (hepatic perfusion amplitude), Cdz / dtmax (hepatic perfusion rate), C / B-C C (fast ejection interval), I ring and total ring were gradually decreased and narrowed, while Hd / Hs (description of hepatic vein, portal vein reflux resistance), Q-B / B-Y (hepatic perfusion resistance) Increased significantly compared with the corresponding health group, the difference was highly significant (P <0.01). Conclusion: Hs, Hd / Hs, Cdz / dtmax, C / B-C, B-C and Q-B / It can objectively and sensitively reflect the hemodynamic status of fatty liver.
