目的总结分析手卫生成本对医院感染发病率的影响,为制定科学的医院感染预防控制措施和加强医院感染管理提供依据。方法回顾性调查2013年1月-2015年12月手卫生的投入成本,以及相对应医务人员手卫生依从率和医院感染发病率。结果 2013-2015年手卫生成本投入分别为1.8065万元、8.9049万元和9.8519万元,出院患者人均成本分别为8.67元、33.71元和29.95元。其手卫生依从率分别为32.5%、46.5%和48.5%,医院感染发病率分别为3.17%、1.51%和1.43%。2015年和2014年与2013年相比,手卫生依从率提升了16%和14%(χ~2=10.624,χ~2=8.202,P<0.01);医院感染发病率下降了1.74%和1.66%(χ~2=19.446,χ~2=14.538,P<0.01)。结论适度增加手卫生成本的投入,可明显提升医务人员手卫生依从率,降低医院感染发病率。
Objective To summarize and analyze the impact of hand hygiene costs on the incidence of nosocomial infections and provide evidences for the establishment of scientific nosocomial infection prevention and control measures and to strengthen the management of nosocomial infections. Methods A retrospective survey was conducted on the input costs of hand hygiene from January 2013 to December 2015 and the corresponding rates of hand hygiene compliance and hospital infection among medical staff. Results The costs of hand hygiene in 2013-2015 were respectively 1.8065 million yuan, 8.9049 million yuan and 9.8519 million yuan. The per capita costs of discharged patients were 8.67 yuan, 33.71 yuan and 29.95 yuan respectively. The hand hygiene compliance rates were 32.5%, 46.5% and 48.5%, respectively, and the prevalence rates of nosocomial infections were 3.17%, 1.51% and 1.43%, respectively. Hand hygiene compliance rates increased by 16% and 14% in 2015 and 2014 compared with 2013 (χ ~ 2 = 10.624, χ ~ 2 = 8.202, P <0.01); the incidence of nosocomial infections decreased by 1.74% and 1.66 % (χ ~ 2 = 19.446, χ ~ 2 = 14.538, P <0.01). Conclusion Appropriate increase in hand hygiene costs of inputs, can significantly improve the medical staff compliance rate of hand hygiene and reduce the incidence of nosocomial infections.
