目的:观察和解调气法联合顺铂胸腔灌注治疗邪犯胸肺型恶性胸腔积液的临床疗效。方法:将54例邪犯胸肺型恶性胸腔积液患者随机分成治疗组和对照组各27例。2组均胸腔内留置中心静脉导管,对照组予单纯胸腔内顺铂灌注治疗,治疗组在对照组基础上加服和解调气中药汤剂小柴胡汤治疗。观察近期疗效、中医证候疗效、生活质量、毒副反应等。结果:治疗组脱落1例,对照组脱落2例。治疗组胸水有效率、胸水缓解稳定率分别为73.08%、96.15%,对照组分别为68.00%、76.00%,2组胸水有效率差异无统计学意义(P>0.05),胸水缓解稳定率差异有统计学意义(P<0.05);中医证候疗效总有效率治疗组为84.62%,对照组为72.00%,2组差异有统计学意义(P<0.05);治疗组在卡氏评分、毒副反应发生情况方面均优于对照组,差异有统计学意义(P<0.01或P<0.05)。结论:和解调气法联合顺铂胸腔灌注对于恶性胸腔积液邪犯胸肺型患者,与单纯西医治疗比较虽有效率无显著差异,但可减缓胸水进展、改善症状、提高生活质量,并减轻化疗药物毒副反应。
Objective: To observe and solve the clinical effect of gas therapy combined with cisplatin pleural perfusion in treatment of malignant pleural effusion and malignant pleural effusion. Methods: 54 cases of malignant pleural effusion malignant pleural effusion were randomly divided into treatment group and control group of 27 cases. Both groups were placed in the central venous catheter for thoracic cavity, the control group was treated with Cisplatin in the thoracic cavity. The treatment group was treated with Xiaochaihu decoction, a traditional Chinese medicine decoction, on the basis of the control group. Observation of short-term efficacy, TCM syndrome efficacy, quality of life, side effects and so on. Results: The treatment group was shed in 1 case, the control group was shed in 2 cases. The effective rates of pleural effusion and pleural effusion were 73.08% and 96.15% in the treatment group and 68.00% and 76.00% in the control group respectively. There was no significant difference in the pleural effusion efficiency between the two groups (P> 0.05) (P <0.05). The total effective rate of traditional Chinese medicine syndrome was 84.62% in the treatment group and 72.00% in the control group, with significant difference between the two groups (P <0.05) The response was better than the control group, the difference was statistically significant (P <0.01 or P <0.05). Conclusions: Compared with Western medicine treatment, no significant difference was found between the two methods in treating pleural effusions of chest and lung in patients with malignant pleural effusion. Reduce the side effects of chemotherapy drugs.
