In the past 5 to 10 years, a substantial advance has been made in the clinical translational research of thyroid cancer.MARK and PI3K-AKT pathways are recognized as the main signaling transduction pathways involved in the carcinogenesis of either differentiated thyroid cancer (DTC) or medullary thyroid cancer (MTC).Multiple genetic alterations have been proven to be responsible for the activation of the two pathways.Currently,VEGFR-2, RET, and BRAF are promising therapeutic targets in metastatic thyroid cancer that previously lacked standard treatment options.For patients with radioactive-iodine refractory DTC that progressed within 14 months before enrollment, a phase Ⅲ trial (DECISION study) was reported this year during the ASCO annual meeting.Compared with placebo, sorafenib was associated with an almost doubled median, progressionfree survival (PFS, 10.8 vs.5.8 months, P<0.0001) and a much higher disease control rate (DCR, 54.1% vs.33.8%, P<0.0001).
