微等离子体作用于皮肤组织的动物实验研究

  目的:明确微等离子体射频技术的组织学作用机理,评价不同能级及治疗技术上的差异带来的组织学差异。从而为临床制定最佳治疗方案提供理论支持。方法:选择两只猪龄5-6个月白色小型猪,于猪背皮肤上用12组不同参数的微等离子体射频治疗。(包括不同的治疗能级,持续时间,治疗遍数,滑动速度及治疗密度。)两只动物治疗参数完全一致。分别于治疗后即刻、治疗后7天、1个月、3个月及6个月观察治疗区域皮肤的外观,愈合情况,判断是否遗留瘢痕。并取皮肤病理组织活检,用HE染色及Masson三色染色后评估治疗区域皮肤的损伤形态、治疗深度及热损伤深度,组织愈合的速度及胶原新生的过程等。并应用免疫组化的方法测定热休克蛋白的产生及变化过程。比较不同的治疗参数下组织学变化的差别。结果:通过愈合过程和组织学切片观察,滚轮治疗头是局灶性损伤,类似于"点阵激光"治疗较均匀,愈合较快,适合于大面积的覆盖治疗。定点无套筒头组织创伤较大,片状损伤,适用于局部重点治疗,不宜用于大面积覆盖治疗,以免引起过重的皮肤创伤,愈合延迟,甚至导致新的瘢痕。定点有套筒头的组织热效应明显,组织可能伴有不规则点状气化损伤,愈合速度亦较快,治疗反应有个体差异,考虑与治疗操作手法有一定关系,属于操作者依赖性较强的治疗。结论:微等离子体射频技术是一种新型的局灶性换肤技术,通过等离子体浅表的剥脱效应及射频带来的组织热效应,引起皮肤表层的重构及真皮胶原的增生及重塑,适用于瘢痕及紧肤治疗,临床需根据病情使用不同的治疗头及适宜的治疗参数,以保证治疗的有效性及安全性。
