目的探讨18F-氟代脱氧葡萄糖(18F-fluorodeoxyglucose,18F-FDG)正电子发射断层显像术(positron emission tomography,PET)/计算机体层摄影术(computed tomography,CT,PET/CT)和血清鳞状上皮细胞癌抗原(SCC-Ag)检查在宫颈鳞癌复发或转移中的应用。方法顺序入选山西省肿瘤医院46例宫颈鳞癌术后可疑复发的患者,分别行18F-FDG PET/CT及血清SCC-Ag检查,经过二次手术或穿刺者与病理结果对照,未经手术证实者结合随诊(>6个月)或临床诊断。结果术后可疑存在复发或转移者46例,其中30例患者最终出现复发或转移,16例患者未出现;在30例患者中,18F-FDG PET/CT检出28例,2例假阴性,血清SCC-Ag检查检出18例,12例假阴性;在16例未出现复发或转移的患者中,18F-FDG PET/CT出现1例假阳性,血清SCC-Ag检查出现1例假阴阳性。结论18F-FDG PET/CT与血清SCC-Ag检查均是检测宫颈鳞癌术后复发或转移的有效方法,但血清SCC-Ag检查较PET/CT检查假阴性率高;宫颈鳞癌术后患者应定期行18F-FDG PET/CT检查,尤其是血清SCC-Ag升高的患者。
Objective To investigate the diagnostic value of 18F-fluorodeoxyglucose (18F-FDG) positron emission tomography (PET) / computed tomography (CT) Application of squamous cell carcinoma antigen (SCC-Ag) in the recurrence or metastasis of cervical squamous cell carcinoma. METHODS: Forty-six patients with suspected squamous cell carcinoma of the cervical squamous cell carcinoma were enrolled in Shanxi Cancer Hospital. The patients were examined by 18 F-FDG PET / CT and SCC-Ag respectively. The patients underwent secondary surgery or puncture were compared with the pathological findings without surgical confirmation Combined with follow-up (> 6 months) or clinical diagnosis. Results Of the 46 patients with suspected recurrence or metastasis, 30 patients eventually developed recurrence or metastasis, and 16 patients did not appear. Of the 30 patients, 28 were detected by 18F-FDG PET / CT, and 2 were false-negative and serum 18 cases were detected by SCC-Ag, and 12 cases were false negative. In 16 cases of patients without recurrence or metastasis, one case of false positive of 18F-FDG PET / CT and one case of false negative of serum SCC-Ag were found. Conclusion Both 18F-FDG PET / CT and serum SCC-Ag are effective methods for the detection of recurrence or metastasis of cervical squamous cell carcinoma, but the false-negative rate of serum SCC-Ag is higher than that of PET / CT. 18F-FDG PET / CT should be performed on a regular basis, especially in patients with elevated serum SCC-Ag.
