目的探讨支原体肺炎(MP)患儿心血管系统损害的临床特点。方法选取本院2013年1月至2016年1月收治的115例MP感染并发心血管系统损害患儿为研究对象,针对患儿开展心电图、胸部X线片、血支原体、心肌酶、C反应蛋白等临床检查,并给予阿奇霉素、辅酶QA、三磷酸腺苷、黄芪与维生素C等药物治疗,观察本组患者的临床检验结果与治疗效果,并分析支原体肺炎并发心血管系统损害患儿的临床特点。结果本组患儿中WBC偏高51例,ESR升高79例,CPR升高74例,2项以上心肌酶升高115例,血清肌钙蛋白升高71例,心肌抗体阳性70例,胸部X线片表现异常60例,胸部CT检查表现异常40例,心脏B超检查提示心脏增大心包积液6例,心事扩大6例,先天性心脏病11例。经临床治疗后,心肌酶正常105例,好转10例,心功能均明显好转,1例伴阿斯综合征患儿死亡。结论 MP感染会引起多脏器损害,尤其是心血管系统损害,经心功能相关检查可获得良好诊断效果,临床上应做到早诊断与早治疗,以降低并发症发生率,促进患儿预后。
Objective To investigate the clinical features of cardiovascular system damage in children with mycoplasma pneumonia (MP). Methods From January 2013 to January 2016 in our hospital, 115 cases of MP-infected children with cardiovascular system injury were enrolled in this study. The electrocardiogram, chest X-ray, blood mycoplasma, myocardial enzymes, C-reactive protein And other clinical tests, and given azithromycin, coenzyme QA, adenosine triphosphate, astragalus and vitamin C and other drugs, the clinical observation of the patients and treatment results, and analysis of mycoplasma pneumonia complicated with cardiovascular system in children with clinical features. Results In this group, WBC was elevated in 51 cases, ESR increased in 79 cases, CPR increased in 74 cases, 2 or more myocardial enzymes increased in 115 cases, serum troponin increased in 71 cases, myocardial antibodies in 70 cases, chest X-ray abnormalities in 60 cases, thoracic CT examination showed abnormalities in 40 cases, heart B-ultrasound showed increased cardiac pericardial effusion in 6 cases, cardiac expansion in 6 cases, 11 cases of congenital heart disease. After clinical treatment, the normal myocardial enzyme in 105 cases, improved in 10 cases, cardiac function were significantly improved, 1 case of children with Asperger’s death syndrome. Conclusion MP infection can cause multiple organ damage, especially cardiovascular damage, cardiac function related tests can get good diagnostic results, clinical diagnosis and treatment should be done early to reduce the incidence of complications and promote the prognosis of children .
