目的探讨肿瘤坏死因子(TNF-α)、酪酪肽(PYY)、白细胞介素1受体拮抗剂(IL-1ra)与多囊卵巢综合征(PCOS)之间的关系,拟为PCOS的发病机制和治疗提供理论基础。方法选取2013年6月-2016年6月于武汉市红十字会医院妇科就诊的30例PCOS患者设为PCOS组,30例健康女性纳入对照组,采用酶放大免疫发光法检测所有患者的卵泡刺激素(FSH)、黄体生成素(LH)、睾酮(T),酶联免疫吸附法检测患者血清TNF-α、PYY、IL-1ra,分析两组患者各因子水平之间的关系。结果 PCOS组患者的LH、T、TNF-α、IL-1ra水平均显著高于对照组,差异均有统计学意义(均P<0.05),FSH、PYY水平显著低于对照组,差异均有统计学意义(均P<0.05);PCOS组患者的TNF-α、IL-1ra水平与LH/FSH及T水平具有正相关性(均r>0,P<0.05),但PYY和LH/FSH及T水平呈负相关性(r=-0.396,P<0.05),PCOS组患者的TNF-α、IL-1ra与PYY水平之间呈负相关性(均r<0,P<0.05),但TNF-α和IL-1ra水平呈正相关性(r=0.502,P<0.05);对照组的TNF-α、IL-1ra、PYY与LH/FSH及T水平无明显相关性(均P>0.05)。结论 PCOS的发生可能和亚临床慢性炎症有关,同时也可能和胃肠系统的分泌素PYY相关。
Objective To investigate the relationship between TNF-α, PYY, IL-1ra and PCOS in patients with PCOS Mechanism and treatment provide the theoretical basis. Methods From June 2013 to June 2016, 30 patients with PCOS in gynecology department of Wuhan Red Cross Hospital were enrolled as PCOS group and 30 healthy women were enrolled in the control group. Enzyme-enhanced immuno-luminescence was used to detect the follicle stimulating Serum levels of TNF-α, PYY and IL-1ra were detected by enzyme-linked immunosorbent assay (FSH), LH, T and ELISA. Results The levels of LH, T, TNF-α and IL-1ra in PCOS group were significantly higher than those in control group (all P <0.05), while the levels of FSH and PYY were significantly lower than those in control group The levels of TNF-α and IL-1ra in patients with PCOS were positively correlated with the levels of LH / FSH and T (all r> 0, P <0.05), but the levels of PYY and LH / FSH (R = -0.396, P <0.05). The levels of TNF-α and IL-1ra in patients with PCOS were negatively correlated with PYY (all r <0, P <0.05) TNF-α and IL-1ra levels were positively correlated (r = 0.502, P <0.05). There was no significant correlation between TNF-α, IL-1ra and PYY and LH / FSH and T levels in the control group (all P> 0.05) . Conclusions The occurrence of PCOS may be related to subclinical chronic inflammation, and may also be related to secreted PYY in the gastrointestinal system.
