目的探讨尖锐湿疣(CA)患者人乳头瘤病毒(HPV)-DNA定量及分型。方法采用荧光定量聚合酶链反应(FQ-PCR)对278例CA患者疣体组织进行HPV分型和HPV-DNA定量检测。结果 278例CA患者经二氧化碳激光治疗后173例治愈,105例复发。HPV亚型分析显示,278例患者的标本中均检出HPV-DNA(100.00%),244例(87.77%)基因型为HPV6/11,7例(2.52%)为HPV16/18,27例(9.71%)为HPV6/11+16/18。对278例HPV-DNA阳性患者治疗前病毒载量进行分组比较,病毒载量为103~105拷贝/ml的患者106例,有23例患者复发;病毒载量105~107拷贝/ml的患者82例,有29例患者复发;病毒载量>107拷贝/ml的患者90例,有53例患者复发。HPV亚型为HPV6/11的有79例复发,HPV16/18的有5例复发,HPV6/11并发HPV16/18的有21例复发。结论检测CA患者皮损HPV-DNA能区分高危型和低危型HPV感染,可对CA复发进行预测。
Objective To investigate the quantitative and typing of human papillomavirus (HPV) -DNA in patients with condyloma acuminatum (CA). Methods FQ-PCR was used to detect HPV type and HPV-DNA in wart tissues of 278 CA patients. Results 278 cases of CA patients cured by carbon dioxide laser 173 cases were cured, 105 cases of recurrence. HPV subtype analysis showed that HPV-DNA (100.00%) was detected in 278 patients, 244 (87.77%) were HPV 6/11, 7 (2.52%) were HPV 16/18, and 27 9.71%) was HPV6 / 11 + 16/18. A total of 278 HPV-DNA-positive patients were divided into two groups according to their viral load before treatment: 106 patients with viral load of 103-105 copies / ml and 23 patients relapsed. Patients with viral load of 105-107 copies / ml Of the 29 patients who relapsed, 90 patients had viral load> 107 copies / ml and 53 patients relapsed. There were 79 recurrent HPV subtypes HPV6 / 11, 5 recurrent HPV16 / 18 and 21 recurrent HPV6 / 11 HPV16 / 18. Conclusion The detection of HPV-DNA in lesions of CA patients can distinguish high-risk and low-risk HPV infections and predict the recurrence of CA.
