目的探讨通过使用微量脉冲泵皮下泵入戈那瑞林治疗低促性腺激素性性腺功能减退症(HH),研究促性腺激素释放激素(Gn RH)泵在HH治疗中的作用。方法通过报道1例使用Gn RH泵治疗HH患者疗效及复习相关文献,研究Gn RH泵治疗与传统方式治疗对HH患者的性激素水平、第二性征发育、生精及受孕率等情况影响。结果经治疗,Gn RH泵有效地改善了患者的性激素水平、第二性征发育及生精能力,并最终达到生育目的。患者声音变调、腋毛及胡须长出,有性冲动,情绪及心态好转,促黄体生成素、卵泡刺激素及睾酮均正常,精子密度为20×109/L,平均精子活力>30%。结论脉冲泵入Gn RH更符合生理性Gn RH的分泌节律,疗效及耐受性好,故推荐Gn RH泵为HH治疗的优先选择。
Objective To investigate the effect of gonadotropin-releasing hormone (Gn RH) pump on the treatment of HH by subcutaneously pumping gonadorelin into hypogonadotropic hypogonadism (HH) with micro-pulse pump. Methods One case of Hn patients treated with Gn RH pump and the related literatures were reviewed. The effects of Gn RH pump therapy and traditional treatment on the levels of sex hormones, development of second sexual characteristics, spermatogenesis and pregnancy rate in HH patients were studied. The results of treatment, Gn RH pump effectively improve the patient’s sex hormone levels, secondary sexual characteristics and spermatogenesis, and ultimately to achieve reproductive goals. Patients with tone changes, armpit hair and beard grow, sexual impulses, mood and mood improvement, luteinizing hormone, follicle stimulating hormone and testosterone were normal sperm density of 20 × 109 / L, the average sperm motility> 30%. Conclusion Pulsed pump Gn RH is more consistent with physiological Gn RH secretion rhythm, efficacy and tolerability, it is recommended Gn RH pump for the treatment of HH preferred.
