目的:分析南方某中学阑尾炎爆发病例的特征以及爆发的原因。方法:调查阑尾炎爆发的人群和时间分布及暴露因素,复习爆发病例的病历和病理切片。结果:2005年1月-2006年12月间该校出现阑尾炎病例36名,男女生的发病率分别为1.9%和8.6%(Z=3.67,P<0.01)。男女生的生活习惯不同。病例常成簇出现,病例间常有接触史。极少数病例体温、白细胞和中性粒细胞百分比增高。所有病例组织学诊断为急性单纯性阑尾炎,组织学改变的特征为阑尾组织出血、淋巴细胞和嗜酸细胞浸润。结论:爆发病例是“非穿孔性阑尾炎”实体,可能由传染因子所致。
Objective: To analyze the characteristics of the appendicitis outbreak in a middle school in southern China and the causes of the outbreak. Methods: To investigate the distribution and exposure of appendicitis to outbreaks of time, the review of medical records and pathological sections of outbreaks. Results: From January 2005 to December 2006, 36 appendicitis cases were found in the school. The incidence of male and female students was 1.9% and 8.6% respectively (Z = 3.67, P <0.01). Different habits of boys and girls. Clusters often occur, the case often have contact history. Very few cases of body temperature, white blood cells and neutrophils increased the percentage. All cases were histologically diagnosed as acute simple appendicitis and histological changes were characterized by bleeding in the appendix, lymphocytic and eosinophilic infiltrates. Conclusions: Outbreaks are “non-perforated appendicitis” entities that may be caused by infectious agents.
