目的探讨血清促甲状腺激素(TSH)是否与胰岛素存在相关性。方法对某地区822名健康体检者进行问卷调查,并测试血清TSH、血糖、血脂、真胰岛素(TI)等,根据HOMA-IR的上1/4位点将资料分为非胰岛素抵抗组和胰岛素抵抗组;并按TSH的正常范围将TSH分为三组:TSH正常组、TSH高水平组、TSH低水平组。并进行相关分析。结果 (1)血清TSH水平与胰岛素抵抗存在一定关系,在胰岛素抵抗组明显高于非胰岛素抵抗组(P<0.05);血清TSH水平与TI正相关(β=0.057,P=0.024)。(2)血清TSH水平与BMI相关(β=0.203,P=0.003),血清TSH水平与TI相关(β=0.044,P=0.043),血清TSH水平与血压、腰围未见明显相关(P>0.05)。(3)多元Logistic回归分析表明,与正常组比较,TSH高水平组的胰岛素抵抗的发病危险比率升高。结论 (1)血清TSH水平与胰岛素有一定关系。TSH在胰岛素抵抗组明显高于非胰岛素抵抗组。(2)监测血清TSH的水平变化可能对胰岛素抵抗及肥胖有一定的指导意义。
Objective To investigate whether serum thyrotropin (TSH) is related to insulin. Methods A total of 822 healthy volunteers in a certain area were surveyed and the serum TSH, blood glucose, blood lipids and true insulin (TI) were tested. According to the upper 1/4 of HOMA-IR, the data were divided into non-insulin resistance group and insulin Resistant group. According to the normal range of TSH, TSH was divided into three groups: TSH normal group, TSH high level group and TSH low level group. And make relevant analysis. Serum TSH levels were positively correlated with TI (β = 0.057, P = 0.024) .Conclusion (1) Serum TSH level has a certain relationship with insulin resistance, which is significantly higher in insulin resistance group than non-insulin resistance group (P <0.05). (2) Serum TSH level was correlated with BMI (β = 0.203, P = 0.003), serum TSH level was correlated with TI (β = 0.044, P = 0.043), serum TSH level was not significantly correlated with blood pressure and waist circumference ). (3) Multivariate Logistic regression analysis showed that, compared with the normal group, the risk of insulin resistance in the high TSH group increased. Conclusion (1) Serum TSH levels and insulin have a certain relationship. TSH in insulin resistance group was significantly higher than non-insulin resistance group. (2) Monitoring serum TSH levels may have some guiding significance on insulin resistance and obesity.
