目的 :探讨放化综合治疗由恶性肿瘤导致的上腔静脉综合征 ( SVCS)的临床疗效。方法 :对 2 2例患者采用放化综合治疗。放疗采用 6MV X线外照射 ,先大剂量冲击照射 ,4Gy/次 /d,连续 3~ 5 d后改用常规分割照射。化疗用 EP、NP或 AFP方案 ,恶性淋巴瘤采用CHOP方案。结果 :2 2例中 1 4例完全缓解 ( CR) ,6例部分缓解 ( PR) ,总有效率 90 .9%。其中小细胞肺癌及恶性淋巴瘤无论先放疗或先化疗均达 CR。结论 :大剂量冲击放疗联合化疗对于缓解上腔静脉压迫疗效确切 ,值得临床推广。
Objective: To investigate the clinical effects of radiotherapy and chemotherapy in the treatment of superior vena cava syndrome (SVCS) caused by malignant tumors. Methods: 22 cases of patients with radiotherapy and comprehensive treatment. Radiotherapy using 6MV X-ray external irradiation, the first large-dose impact irradiation, 4Gy / time / d, continuous 3 ~ 5 d switch to conventional split irradiation. Chemotherapy with EP, NP or AFP program, malignant lymphoma using CHOP program. Results: Twenty-two of 14 patients had complete remission (CR) and 6 patients had partial response (PR), with a total effective rate of 90.9%. The small cell lung cancer and malignant lymphoma, regardless of the first radiotherapy or chemotherapy were up to CR. Conclusion: The combination of high-dose shock radiotherapy and chemotherapy is effective in relieving the superior vena cava compression and is worthy of clinical promotion.
